Why this role matters 

Homelessness in Central Australia is shaped by complex social, cultural and structural realities. Many people experience unstable housing alongside mental health challenges, alcohol and other drug concerns and involvement with the justice system. Access to consistent support can be limited, and the work of our frontline teams truly matters. 

As a Case Manager at Open House, you will walk alongside men referred through the justice system, including those on bail or parole, helping them stabilise, meet their conditions and work towards long term housing and wellbeing goals. You will create a safe, structured and supportive environment where participants feel respected, understood and empowered. Your work directly contributes to improved outcomes for individuals and strengthens the broader community.

About the role 

You will provide case management to participants living in the Men’s Open House supported accommodation program. Working closely with the Team Leader and a multidisciplinary team, you will undertake intake and assessment, develop collaborative case plans and support participants to achieve self-determined goals. 

Your role includes advocating for clients, assisting them to access services, supporting appointments and maintaining accurate records in line with TSA standards. You will identify risks, respond appropriately to high-risk situations and ensure case management aligns with the National Homelessness Stream Model of Care. 

This full-time permanent position reports to the Team Leader and pay is in accordance with a SCHADS Level 4. 

How you will make an impact 

  • Undertake intake and assessment to build a shared understanding of participant needs and strengths 

  • Develop individual case plans in collaboration with participants 

  • Support participants to meet bail or parole conditions and work towards long term goals 

  • Assist participants to access services, attend appointments and build community supports 

  • Advocate on behalf of participants with government and non-government agencies 

  • Maintain accurate, up to date client records and data in line with TSA standards 

  • Monitor progress and review case plans to ensure they continue to meet participant needs 

  • Identify risks and escalate concerns to the Team Leader as required 

  • Contribute to a safe, structured and supportive environment within the residential facility 

What you will bring 

  • Certificate IV in Community Services or a related qualification 

  • Experience supporting people in a social services environment 

  • Demonstrated case management experience and skills 

  • Case management skills grounded in empathy, structure and good judgement 

  • Confidence working with people from diverse cultural backgrounds 

  • Understanding of local Aboriginal communities and culture 

  • Ability to remain calm, resilient and responsive in challenging situations 

  • Commitment to culturally safe, inclusive and person-centred practice 

  • NT Driver’s Licence 

  • NT Working with Children Check 

What we offer 

  • $51.37 – $55.28 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging 

  • SCHADS Level 4 – Above Award conditions (up to 10%)
