About this role:
As an Advanced Practice Provider in Family Medicine, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will work with 4 physicians and 3 advanced practice providers who place high importance on teamwork and collaboration. Our team is well-known for synergy, and high patient satisfaction scores.
You will practice at Hurricane Valley Clinic in Hurricane, UT
Your hours will be full-time, and your schedule may be 3 to 4 days per week depending on the clinic's needs. Our clinic is open Monday – Friday 7am – 7pm weekly.
Your on-call responsibilities are shared among all providers and will be approximately every 7th week for 7 days.

What you’ll bring:
APRN or PA-C degree from an accredited program
Board certification
Active, unobstructed Utah medical license, or the ability to obtain one
Full prescriptive authority and an active DEA, or the ability to obtain one
Ability to successfully complete Intermountain Health’s credentialing process
2+ Years of experience is preferred
BLS Certification
