Conviva Senior Primary Care, a subsidiary of Humana Inc., is recruiting a full-time primary care physician for our Ormond Beach, Florida center. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director.The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy.
A day in the life of a Conviva Primary Care Physician:
- Monday-Friday 8:00am-5:00pm
- Join an experienced team with an established patient panel
- Opportunity to spend more time with patients
- Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care
- Minimal, shared call rotation

Required Qualifications
- Doctor of Medicine (MD) or Osteopathic (DO)
- Board certified or board eligible in Family Medicine or Internal Medicine - Geriatrics specialty would be ideal
- Valid, unrestricted license (or willing to obtain) in the state of desired employment (Florida)
- Current, unrestricted DEA license
Preferred Qualifications:
- Minimum of two to five years directly applicable experience preferred
- New graduates will be considered
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Bilingual (English and Spanish) is a plus

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
