Responsibilities
As a Physical Therapist, you will:
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services in a skilled nursing and post-acute setting
- Improve patient mobility, strength, balance, endurance, and functional independence
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals
- Educate patients and families regarding treatment goals, recovery expectations, and home exercise programs
- Complete accurate and timely clinical documentation in compliance with state and federal regulations
- Contribute to positive patient outcomes and quality-of-care initiatives
Qualifications
- Current and unrestricted California Physical Therapist License required
- Graduate of an accredited Physical Therapy program
- Skilled Nursing Facility (SNF), rehabilitation, long-term care, or post-acute experience preferred
- Strong clinical assessment and treatment skills
- Excellent communication, organization, and time-management abilities
- Ability to work independently while contributing to a team-focused environment
- Commitment to delivering patient-centered care
