Role Description

Role Description
The Referral Coordination Team Lead will join Zócalo Health during a period of rapid growth and increasing operational complexity. This role exists to help scale and execute a community-oriented primary care model that delivers measurable outcomes for high-need members and health plan partners.
As part of a fast-growing team, you will be responsible for overseeing referral intake, processing, tracking, and referral-based outreach. This role ensures all incoming referrals are managed accurately, efficiently, and in alignment with program and health plan requirements. The Referral Coordination Team Lead works closely with Community Partnerships Managers to support external referral relationships and ensure a seamless referral experience for partners.

Role Description
The Community Engagement Manager (CEM) is a people leader and operational owner responsible for driving the performance, effectiveness, and accountability of a team of Community Engagement Specialists (CES). This role does not conduct direct outreach. Instead, the CEM is accountable for setting strategy, overseeing execution, ensuring quality, and delivering results across community engagement and patient acquisition efforts.
The CEM leads and manages a team responsible for community-based outreach activities—including events, door-to-door engagement, and neighborhood canvassing—with the goal of enrolling eligible individuals into Zócalo Health’s care programs. The ideal candidate is highly operational and data-driven, bringing structure to field operations, monitoring and auditing work quality, and using data to continuously refine outreach strategy. This role is responsible for recommending and managing regional strategies for how team time and coverage—including evenings and weekends—are allocated, ensuring staff resources are deployed effectively to maximize engagement, enrollment, and return on outreach efforts. The ideal candidate is energized by operating in ambiguity, creating structure where none exists, implementing new processes, defining best practices, and scaling systems in a fast-moving, mission-driven environment. The CEM thrives as a strategic planner, people manager, and systems thinker who can translate goals into clear execution plans and measurable outcomes.

Role Description

As a founding member of the Product team, you will establish the foundational design layer that supports how care is delivered at Zócalo. Today, we do not have a formal design system or documented UX patterns. This role exists to bring structure, clarity, and consistency to our product experience while remaining deeply grounded in real-world care delivery.
You will design workflow-heavy tools used daily by Community Health Workers, clinicians, care coordinators, and operations teams, translating complex, constrained processes into intuitive, usable experiences. Your work spans operational workflow tools and data-facing surfaces that give care teams and leaders visibility into performance. In parallel, you will partner closely with your manager and cross-functional leads to prototype and visualize future-state experiences, helping leadership see where the platform is headed and what to build next

Role Description
The Senior Product Manager, Data, Reporting, and Platform Foundations will join Zócalo Health at a pivotal moment as we invest in the reporting, data, and platform foundations required to scale our care model across plans, programs, and geographies.
This role owns the Data and Reporting product domain end-to-end. As Zócalo scales across plans and payer relationships, reporting, operational visibility, and data infrastructure become foundational to audit readiness, payer trust, and informed decision-making. This role exists to build those capabilities systematically. Beyond reporting, you will define what data the platform needs to support, how data requirements evolve as care operations scale across plans and geographies, and how platform decisions support both near-term execution and long-term strategy.
You will define and drive your own roadmap, partner closely with Engineering to establish clear platform boundaries, data contracts, and decision frameworks, and make principled tradeoffs as ingestion patterns mature. As additional Product Managers join, this role may shift toward systems migration ownership, cross-team operational consistency, and vendor transition management — high-leverage work that directly supports Zócalo’s ability to scale without accumulating operational debt.

Role Description
Zócalo Health is entering a new phase of growth. The company has proven demand for its model and is expanding across markets, populations, and payer relationships. What allowed the organization to move quickly in the early years was strong leaders solving problems locally and responding in real time to emerging needs. That approach worked when the organization was smaller. At scale it becomes fragile.
The next phase of the company requires something different. It requires a durable operating system that allows strong teams to execute consistently across markets without relying on constant escalation or heroics.
The Chief Operating Officer will build that operating system.
This leader will be responsible for how execution happens across the company. The COO will oversee the core care delivery engine of the organization including clinical operations, regional leadership, community health worker teams, patient engagement operations, and CalAIM program delivery.
More importantly, the COO will design the mechanisms that connect these groups so that strategy translates into coordinated action and operational performance is predictable across markets.
The COO will serve as the integrator across the leadership team. This leader will ensure that priorities, resources, and operational execution stay aligned as the organization scales across new markets and payer relationships.
The COO partners closely with the CEO and Chief Strategy Officer to translate company strategy into operational execution. Strategy and Growth define where the organization expands and how it wins in the market. The COO ensures the operating model, organizational structure, and execution systems are in place so those strategic decisions can be implemented consistently across markets.
This role reports directly to the CEO and will play a central role in shaping how Zócalo Health scales its care model into new markets.
In your first 12 months, you will:

Role Description
The Senior EHR Systems Administrator (athenahealth) is the system owner and architectural authority for Zócalo Health’s athenahealth environment. This role is responsible for ensuring the EHR operates as a safe, stable, scalable, and audit-defensible foundation for clinical care, billing, reporting, and operational workflows.
This role owns the governance model, configuration architecture, and long-term structural integrity of athenahealth. The Senior EHR Systems Administrator defines how the system is used, ensures changes are safe and scalable, and prevents system degradation or architectural drift as the organization grows.
This is not a ticket execution or IT support role. This role is accountable for the reliability, compliance, and long-term health of a mission-critical system that underpins clinical operations, revenue integrity, and audit readiness.
This role operates as a cross-functional leader and trusted authority across Clinical Operations, Billing, Compliance, Product, Engineering, and Data.

Zócalo Health is seeking a behavioral health case manager to serve as the primary point of contact for an assigned caseload of ECM members with behavioral health needs (including members with more complex mental health and/or substance use needs). This role provides relationship-based outreach, care navigation, care coordination, and brief skills-based behavioral health support to help members engage in appropriate services and move toward their goals.
Note: This role focuses on engagement, care coordination, and brief support strategies, including referrals and warm handoffs to psychotherapy, psychiatry, and Specialty Mental Health.
This position reports to the Senior Director of Clinical Operations. You will work primarily in your community, with some work-from-home responsibilities.
Experience

Role Description
The Community Engagement Manager (CEM) is a people leader and operational owner responsible for driving the performance, effectiveness, and accountability of a team of Community Engagement Specialists (CES). This role does not conduct direct outreach. Instead, the CEM is accountable for setting strategy, overseeing execution, ensuring quality, and delivering results across community engagement and patient acquisition efforts.
The CEM leads and manages a team responsible for community-based outreach activities—including events, door-to-door engagement, and neighborhood canvassing—with the goal of enrolling eligible individuals into Zócalo Health’s care programs. The ideal candidate is highly operational and data-driven, bringing structure to field operations, monitoring and auditing work quality, and using data to continuously refine outreach strategy. This role is responsible for recommending and managing regional strategies for how team time and coverage—including evenings and weekends—are allocated, ensuring staff resources are deployed effectively to maximize engagement, enrollment, and return on outreach efforts. The ideal candidate is energized by operating in ambiguity, creating structure where none exists, implementing new processes, defining best practices, and scaling systems in a fast-moving, mission-driven environment. The CEM thrives as a strategic planner, people manager, and systems thinker who can translate goals into clear execution plans and measurable outcomes.
This position will report to the Regional General Manager.

Role Description
The Medical Billing Specialist (CPC) will join Zócalo Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing, and Zócalo Health will lead the transition from fee-for-service to value-based care for the growing Latino community in the U.S.
An integral member of the Revenue Cycle Management team, you will be responsible for supporting accurate, compliant, and timely medical billing across Primary Care, Urgent Care, and Behavioral Health services, including Medi-Cal managed care plans and Medi-Cal initiatives such as Enhanced Care Management (ECM), Community Health Worker (CHW), and Care Services (CS). This role applies certified coding expertise to ensure claims are reviewed, coded correctly, submitted in a timely manner, and followed through to resolution, including denial management. You will collaborate closely with Revenue Cycle stakeholders to support reimbursement accuracy, identify trends impacting claims performance, and contribute to continuous improvement as Zócalo Health scales value-based care for the communities we serve.
This position reports to the Revenue Cycle Management Team Lead.
