The team of drivers and dispatchers the Transportation Supervisor oversees are crucial in maintaining our day-to-day activity.
Reporting to the Area Manager, Transportation, the Transportation Supervisor manages the transportation department staff, coordinates all the transportation services provided to our participants and potential enrollees, and oversees vehicle safety.

Our values and participant focus lead the way no matter what. The WelbeHealth Social Work Assistant acts as an integral part of the IDT that provides direct care to our participants. WelbeHealth will provide you with the team and guidance you need to help our participants achieve their full potential.
The Social Work Assistant provides assistance and manages a caseload of participants working in close coordination with the IDT and Social Worker. The Social Work Assistant functions as a liaison between the participants, their family members, and the IDT by offering advice, implementing treatment plans, coordinating community services, and completing necessary paperwork.

Primary Care Provider
WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with the opportunity to continue living in their homes and in their communities. Our innovative and comprehensive range of medical services to participants is what ignites our passion to treat the whole person and not just the symptoms! We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care.
At the direction of the Medical Director, the Primary Care Provider works collaboratively with WelbeHealth Physicians and the IDT to complete assessments, plan primary care, and educate WelbeHealth participants and their families.
