The Home Care Coordinator plays a vital role by conducting in-home care assessments, setting the framework for our home care team to help our participants thrive. Reporting to the Home Care Manager, the Home Care Coordinator focuses on arranging, assessing, and overseeing personal care in participants’ homes.
This role is different because Home Care Coordinators at WelbeHealth:
- Enjoy a flexible workday, with the freedom to structure assessments based on your desired schedule
- Work a hybrid schedule, combining remote work with required onsite presence for interdisciplinary team (IDT) meetings and home assessments
- Collaborate closely with a supportive home care team and IDT, fostering partnership and teamwork to deliver exceptional participant care

On the day-to-day, you will:
- Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, participant questions, and referrals
- Communicate with participants by phone and coordinate effectively with nursing, therapy, aides, social services, and physicians regarding schedule changes, test results, and other related updates
- Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling hours
- Partner with the Marketing team to support the enrollment of prospective participants into the program
- Assist with staffing and scheduling activities, soliciting, and manager input
- Participate in end-of-life care, coordination, and support
Job requirements include:
- Healthcare/medical licensure or equivalency, along with at least three (3) years of professional experience
- Bachelor’s degree preferred
- Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population
- Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions preferred

The Home Care Scheduler plays a critical role in Home Services, ensuring that the needs of our participants are met with the highest level of care.
Reporting to the Home Care Supervisor, the Home Care Scheduler coordinates the scheduling and tracking of Home Health and Home Care Assistant assignments. The Home Services Scheduler will provide outstanding customer service to all stakeholders (including participants, families, vendors, and staff) and maintain records of all scheduled appointments.
Essential Job Duties:
- Coordinate the scheduling Home Care Assistant/Personal Care Assistant assignments as directed by the Home Health RNs and Home Care Supervisor, including schedule changes and communicating with all stakeholders (participants, family, home services staff, etc.)
- Complete required authorizations in electronic medical records (EMR) system for approved personal care and home health services
- Ensure all medical records for clinicians, home care aids, vendors, and in-home staff visits are received and uploaded into the electronic health record (EHR)
- Consistently implement and improve procedures, such as making reminder calls, clarifying instructions, or sending necessary materials, to ensure a successful experience for providers, staff, and participants
- Maintain current and accurate records of all scheduled home health appointments and home and personal care assignments, detailing both the scheduled times and outcomes
Job Requirements:
- High school diploma or equivalency
- Minimum of one (1) year of experience as a scheduler in a home health or home care agency
- Minimum of one (1) year of experience working with an EMR and scheduling system preferred

This role is different because Home Care Assistants at WelbeHealth:
- Work a stable, full-time schedule – unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances

- Healthcare/medical licensure or equivalency, along with at least three (3) years of professional experience

- Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions preferred

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.
Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.
This role is different because Home Care Assistants at WelbeHealth:
- Work a stable, full-time schedule – unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances
- Visit 2-5 patients per day depending on participant needs and care plans, working shorter individual shifts that still add up to an 8-hour workday
- Receive reimbursement for travel time and mileage between participants

On the day-to-day, you will:
- Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including but not limited to bathing, dressing, grooming, dressing, toileting, and physical lifting when transferring participants
- Assist participants with Instrumental ADLs, including but not limited to laundry, light housekeeping, shopping, and light meal preparation
- Measure and document participants’ vital signs as instructed
- Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes
- Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed
- Provide translation services as needed
Job requirements include:
- High school diploma or equivalency
- Current Basic Life Support (BLS)/First Aid Certification required
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
- Bilingual preferred
- Must be registered with Home Care Registry prior to start date
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
