Job Purpose:
Provide integrated support to patients and physicians by providing a hands-on approach to improve the quality of patient care. This is accomplished by developing personalized relationships with patients and prescribers of assigned clinics to improve medication adherence and promote evidence-based care expanding into various disease states therapy management.
Job Duties:
The Ambulatory Clinical Pharmacist (ACP) will help advance quality pharmacotherapy care by actively assuring the provision of safe, efficacious, and cost-effective drug therapy. The ACP collaborates closely with the hybrid ACP team to maximize full suite of therapy management (TM) clinical services within supported clinics. The pharmacist performs in-person and/or virtual/phone assessments on disease state management and drug therapy education stratified based on medication risk level and approved clinic protocol. The Patient Engagement Specialist will be responsible for providing service for inbound pharmacy phone calls and/or placing outbound patient refill calls and assisting the patient through the entire refill process. The Patient Outreach Supervisor will be a front-line manager for the Patient Outreach team, a team dedicated to communicating with potential patients about the value of hospital base pharmacies. Job Purpose:
Within this role, this person will provide patient care by removing the barriers to medication access. Working with insurance companies and clinician offices to facilitate approval of medications through prior authorizations. Job Purpose:
Our Implementation Team is responsible for turning signed contracts with hospital partners into fully-operational, high-functioning special pharmacy programs. To do this, the Implementation Team works with each hospital partner to fully establish the program, which includes building relationships across the hospital (executives, clinical leadership, physicians, et al), evaluating existing workflows, developing and implementing new processes, working with one or more pharmacies, connecting the hospital site to the Shields central hub, hiring and training team members, communicating then supporting the roll-out of the program, and many other activities.
The Implementation Director is responsible for all efforts related to the company's implementation efforts with hospital partners. This person also helps to expand the breadth of the relationship while working with the hospital partner, so experience with account management and relationship-based selling are helpful. Given the close working relationships and the process/operational work that is required for a successful roll-out, there is significant travel to the hospital partner's location, typically 3-4 days per week.

This role will be responsible for some or all of the below types of activities: responding to patient requests and inquiries regarding prescription drug plan coverage and fulfillment through the pharmacy; inbound calls to support the health systems needs in patient; status checks on prescriptions; assists pharmacies to properly adjudicate electronic prescription drug claims and resolve adjudication error messages; responsible for providing knowledgeable and courteous customer assistance and response to members, providers, and pharmacy team member inquiries; serves as a point of contact between patients, pharmacy, liaisons, clinical providers, and various internal functional areas.

Required Licensure/Certification: We are seeking experienced pharmacy technicians who are state registered as a pharmacy technician. Nationally certified as a CPhT or equivalent is strongly preferred but not required. Should additional licensure need to be obtained, the applicant will commit to obtaining the required licensure within a mutually agreed upon timeframe. Continued employment is contingent upon meeting these requirements.
*If the candidate's state of residence does not have a state registration available, then the National certification is required at time of hire. Should additional licensure need to be obtained, the applicant will commit to obtaining the required licensure within a mutually agreed upon timeframe. Continued employment is contingent upon meeting these requirements.
Experience and Education Requirements:
Education: High school diploma or GED required
Skills: Strong interpersonal communication skills, exceptional phone skills, comfortable with proactively engaging patients telephonically, comfortable reaching out to on-site clinicians, payors, and pharmacies telephonically, highly proficient in Microsoft Office particularly Excel and Word; able to quickly learn other programs, adapt to Shields’ unique model / tools and able to extract relevant information; strong organizational skills a must
Traits: Organized, high-integrity, high accountability, attention to detail, dependable, quality focus, empathetic, good listener/communicator, collaborative, consistently arrives to work on-time with minimal unscheduled time off
