About the Role
Responsibilities
Essential Duties:
- Serve as the primary contact for incoming inquiries, responding to calls from prospective participants and conducting timely follow-ups to support enrollment.
- Clearly communicate the Seen Health PACE enrollment process, ensuring a high conversion rate from inquiry to in-home visit.
- In collaboration with the Eligibility Specialist and Intake Specialist, address financial eligibility concerns, including Medi-Cal questions, Share of Cost (SOC) issues, and related documentation.
- With adequate training and support, perform Medi-Cal information review, application, and follow-up with DPSS until the Medi-Cal case is fully resolved.
- Demonstrate a comprehensive understanding of the MOE Playbook and apply its principles to daily operations.
- Independently schedule and facilitate Enrollment Conferences, ensuring new participants are guided through all required steps and supported throughout the initial assessment process.
- Manage scheduling for participant tours, in-home assessments, Level of Care (LOC) visits, SW and PCP assessments, Enrollment Conferences, and associated transportation.
- Support the Growth Lead in facilitating MOE team meetings, including tracking
assignments, documenting follow-ups, and logging action items in Orbit.
Outreach & Event Responsibilities:
- Represent Seen Health at community events coordinated by outreach specialists, including health fairs, mixers, and provider meetings.
- Assist in the setup, organization, and staffing of events to ensure a positive and engaging experience for attendees.
- Collect participant information during events and enter accurate data into the Orbit system, assigning leads to the appropriate Outreach Specialists.
- Educate seniors and referral partners about the PACE program and its benefits during community outreach efforts.
- Monitor inventory and prepare marketing materials and giveaways for events, ordering new supplies as necessary.
- Maintain a professional and approachable demeanor when interacting with the public at outreach events.
General Responsibilities:
- Ensure timely and accurate completion of all required documentation.
- Maintain confidentiality of participant and company information in accordance with HIPAA and company policies.
- Actively participate in team meetings, quality improvement activities, and staff training. Keep supervisors informed of work.
- Adapt to changing business needs and take on additional responsibilities as assigned.
Qualifications
- Education: Associate’s degree in a related field required; Bachelor’s degree preferred.
- Experience: Minimum of 1 years of experience in a similar role, preferably in healthcare,
outreach, or customer service. - Customer Service Skills: Strong interpersonal and communication skills with a
customer-first mindset. - Independence: Ability to manage tasks effectively with minimal supervision and adapt to
shifting priorities. - Transportation: Must have reliable transportation to attend events and conduct outreach in
the community. - Technical Proficiency: Comfortable using Mac OS and Google Workspace (Docs, Sheets,
etc.); open to learning and using new software platforms. - Language Skills: Bilingual proficiency in Mandarin or Cantonese is strongly preferred.
Medical Clearance:
Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Additional screening may be required in order to protect participants and the community from communicable disease outbreaks (e.g., COVID-19).
Regulatory Requirements:
Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.
Location
- Regular travel to different settings in the San Gabriel Valley community.
- In center at Seen Health in Alhambra, CA.

About the Role
Responsibilities
- Coordinate and execute outreach, PR and community engagement activities to ensure consistent achievement of monthly referral, home visit and enrollment goals.
- Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value
proposition. - Establish and maintain best in class relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
- Develop and implement annual marketing plan to achieve monthly enrollment goals with MOE Director.
- Provide education to referral source in the community regarding Seen Health services.
- Demonstrate in-depth knowledge of all relevant components of MOE Playbook.
- Comply with all rules pertaining to allowable and prohibited marketing practices.
- Identify multiple potential opportunities for process improvement.
- Must be willing and have the ability to work a varied schedule that may include evening nights, weekends and overtime.
- Complete all required documentation and generate reports in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow Seen Health policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Qualifications
Education/Training/Certifications:
- Bachelor’s Degree Marketing or Health Care Administration preferred.
- DHCS Training and Exam as a marketing representative within 30 days of hire.
- Valid State Driver’s License with a clean DMV record.
Skills and Experience:
- Minimum of two years’ experience in a successful sales and marketing role with at least one year in healthcare or senior care setting. Within service area preferred.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment
and make sound decisions in emergency situations. - Patience, with a “can do” attitude.
- Consistent attention to detail on daily deliverables and small to medium sized projects.
- Familiarity with local community agencies and resources preferred.
- Excellent organizational and communication skills.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.

About this Role
The Senior Care Consultant serves as the first point of contact for prospective Seen Health PACE participants and their families. The ideal candidate is bilingual, quick-thinking, compassionate, and skilled in communicating complex healthcare information in a clear, reassuring manner to seniors and their families.This role is primarily phone-based and focuses on answering inbound calls, following up on leads, and guiding seniors through the PACE enrollment process.
Key Responsibilities
- Answer inbound phone calls from prospective seniors, family members, and referral sources.
- Proactively reach out to seniors or their family members who express interest through our various marketing channels
- Conduct phone-based consultations to explain the PACE program, eligibility requirements, and benefits
- Actively follow up on leads that have not yet been contacted or fully engaged by outreach sales staff
- Build rapport and trust with seniors and caregivers through empathetic, patient, and culturally sensitive communication
- Identify eligibility and next steps to move prospects toward enrollment
- Schedule assessments, tours, or appointments as appropriate
- Accurately document call outcomes, lead status, and follow-up actions in the CRM system
- Collaborate closely with outreach, enrollment, and interdisciplinary care teams to support enrollment goals
- Manage call traffic by triaging incoming calls and directing callers to the appropriate departments
- Meet or exceed individual and team enrollment and follow-up performance targets
- Maintain compliance with PACE, Medicare, Medi-Cal, and organizational guidelines
Required Qualifications
- Language competency requirement:
- Preferred: Mandarin, Cantonese, and English
- Acceptable: Mandarin/English or Cantonese/English
- Proven experience in phone sales, healthcare enrollment, call center, or customer service (senior care or healthcare preferred)
- Strong verbal communication skills with the ability to explain information clearly and persuasively
- Demonstrated ability to think quickly, handle objections, and adapt conversations in real time
- Comfortable speaking with seniors and families with patience, empathy, and professionalism
- Strong follow-up and organizational skills
Preferred Qualifications & Competencies
- Experience working with seniors, PACE programs, Medicare, Medi-Cal, or healthcare-related services
- Familiarity with CRM systems and lead management
- Cultural competence working with Chinese-speaking senior populations
