As a Care Manager in our PACE program, you are the primary coordinator for our participants' care experience. You serve as their advocate, navigator, and the person responsible for making sure their care plan comes to life. Your work ensures participants receive the right services across all settings while maintaining their independence and quality of life.
This role demands exceptional attention to detail, strong communication skills, the ability to track and synthesize information across multiple moving pieces, and persistent, diligent follow-through. The Social Worker plans, organizes and implements social work services to participants and their caregivers in accordance with Seen Health policies and all applicable regulations. As an integral member of an Interdisciplinary Team, the social worker performs psychosocial assessments, develops and implements plans of care, conducts counseling and case management, and facilitates communication between the participant, family, caregivers, PACE staff, and provider support network, as appropriate.
Conduct Social Work assessments to determine the psychosocial needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
Deliver and document social work interventions as agreed upon in the participants’ care plans including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual as well as group counseling and case management.
Completes initial assessments, re-assessments, and care plan updates for participants while continually evaluating the participants' social service needs and caregiver support needs.
Participates in regularly scheduled behavioral management meetings with other mental health professionals.
Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.).
Assists physician, or intermediate care provider, and other team members in understanding the significant social and emotional factors related to participant health problems.
Acts as a resource to other team members and staff regarding topics such as dementia, difficult behaviors, and difficult personalities. Supports the IDT in establishing behavior management plans.
Strive for continuous growth and development of cultural competency exhibiting an understanding, awareness, and respect for diversity.
Maintains accurate and timely documentation and paperwork including participants’ electronic medical records according to policies and procedures.
Provides referral support to community resources and participates in inter-agency coordination of care.
Develops and maintains working relationships with community agencies, such as the Department of Social Services, psychiatric facilities, skilled nursing facilities, hospitals, social agencies, and may participate in hospital and/or SNF discharge planning.
Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants’ support network (family, informal caregivers etc.).
Responds to, investigates, and reports complaints of elder abuse.
Maintains the confidentiality of all company procedures, results and information about participants, clients or families in conformance with Health Insurance Portability and Accountability (HIPAA) principles.
Active contributor and change agent to continuously achieve quality care, participant experience and compliance goals.
Proactively supports participant grievance and incident reporting processes.
Completes timely discharge planning.
Conducts home visits and travel to hospital facilities and/or other community agency, as needed.
Participate in state and federal audit preparation activities.
Performs related duties as assigned.
Master’s degree from a school of social work accredited by the Council on Social Work Education.
Minimum one year experience working with frail and elderly population. (3+ years social work experience preferred).
Experience conducting psychosocial assessments, care planning and case management skills required.
Demonstrated ability to work in a multicultural and multidisciplinary team environment.
Excellent oral and verbal communication and meeting facilitation skills
Previous experience assisting persons with behavioral health & substance abuse issues, preferred.
Bilingual ability preferred (Mandarin, Cantonese and Spanish)
Current CPR certification or the ability to obtain CPR certification is required.

The Recreation Aide will play a vital role in delivering daily activities and social engagement programming for participants at our PACE center. Working closely with the Recreation Therapist and Coordinator, this role will focus on facilitating planned group and individual activities that support cognitive, physical, and emotional wellbeing for older adults. This is a hands-on role ideal for someone with energy, empathy, and a proactive spirit.
Schedule: Monday–Friday, Full-Time (generally 8 AM–4:30 PM) in Alhambra.
Location: Los Angeles required. Ability to commute to Alhambra required.
Activity Execution:
Lead daily planned activities including arts & crafts, group games, reading groups, music therapy, and more.
Provide clear instruction and support for participants during each activity.
Adjust approach to meet the needs of participants with varying cognitive and physical abilities.
Participant Engagement:
Foster an inclusive and welcoming environment where all participants feel seen and valued.
Identify and support participants who may not engage with the primary activity by offering alternate options (e.g., puzzles, word searches).

The Recreation Therapist (RT) will coordinate, plan, supervise, and implement engaging therapeutic and recreational programs and activities for the elderly, as part of a multi-disciplinary team providing all-inclusive PACE care. This role will build a sense of community and warm social atmosphere for the PACE participants by evaluating their needs, developing groups, and individual programming to meet those needs.
Performs duties and responsibilities maintaining adherence to regulatory compliance, Seen Health policies and procedures , environmental safety protocols and quality improvement standards.
Plans and implements group and individual activities to improve or maintain the participants’ cognitive, physical, social, psychosocial, and artistic needs.
Documents participant engagement outcomes in the electronic medical record, alerting the interdisciplinary team to day center absences and/or to other issues that undermine care plan goals. Revise care plans as needed.
Oversees daily flow of activities for dementia specific population to determine if smaller groups, one-on-one programming, or sensory stimulation are needed.
Optimizes the participant experience and participant satisfaction with the coordination of culturally sensitive, inclusive and participant centered activities. Engages language interpreters to co-facilitate activities and special events, as needed.
Organizes community outings and special events, including arranging for participant transportation, entertainment, catering, and involvement of other staff.
Develops a monthly calendar of PACE activities for participants, ensuring that the calendar of events is accessible to participants in multiple languages.
Evaluates plans to promote effective programs according to participant abilities and preferences within daily flow of events.*
Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
Maintains general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash, and overall general budgeting.
Conducts initial in-person comprehensive recreational assessment , semi-annual reassessments and unplanned assessments as needed e.g. change of condition, service determination requests.
Coordinates with the PACE Interdisciplinary Team to develop a comprehensive plan of care for each participant.
Actively participates in team discussions offering feedback, ideas and suggestions, both on a day-to-day basis and in monthly department staff meetings.
Attends and participates in required meetings and trainings.
Maintains and completes charting in participant medical records and reporting according to requirements, including signature and dates on progress notes.
Assists participants with non-skilled activities of daily living (ADL), as needed.
Develops specific therapeutic programs as part of the plan of care, including recreational and social activities.
Communicates with skilled nursing or assisted living facilities the therapeutic needs of participants.
Serves as a resource to staff and coordinates volunteer activities at the PACE Center.
Coordinates with various community resources and partners to enhance recreational programming.
Bachelor’s degree in recreational therapy or related field
Two years’ experience in a social or recreational program preferred, providing and coordinating services for the frail or elderly within a health care setting.
Certified Therapeutic Recreation Specialist (CTRS) by the National Council for Therapeutic Recreation Certification - upon hire.
Proficient computer skills. Experience documenting in Electronic Health Record system preferred.
Valid CA driver’s license and proof of auto insurance required

The Registered Dietitian (RD) is responsible for designing and implementing the nutrition services program working in collaboration with the interdisciplinary team (IDT). The RD conducts individual nutrition assessments, develops personalized nutrition care plans and provides dietary counseling and education to participants and their families.

Develops and implements individualized nutrition care plans based on assessment findings, medical data, and participant preferences.
Collaborate with the interdisciplinary team (IDT) to integrate nutrition care into overall participant care plans and ensure coordinated care delivery.
Provide nutrition counseling and education to participants, families, and caregivers on topics such as meal planning, portion control, and dietary modifications.
Deliver and document direct care nutrition interventions in accordance with primary care provider orders (e.g. provision of nutritional supplements) and/or as agreed upon in the participants’ care plans, including but not limited to implementing and monitoring special diets, and providing nutrition counseling to participants and families.
Monitor participants’ dietary intake, weight changes, and nutritional status regularly, making adjustments to care plans as needed.
Maintains accurate and complete documentation of nutrition assessments, interventions, progress notes, and all communication with participants, healthcare providers and/ or participant caregivers in the participant’s electronic health record.
Oversees menu planning and meal delivery programs, ensuring cultural sensitivity (e.g. posting menus in multiple languages and offering various meal options to satisfy participant preferences).
Supervises compliance with safety and regulatory requirements e.g. prepares and updates nutrition meal cards ensuring that participants receive meals that meet their specific dietary needs and restrictions.
Direct and mentor center staff, including training and overseeing their performance in delivering nutrition services e.g. ensures that all servers are SafeServ certified and demonstrate utilization of nutrition meal cards.
Guide the intake and enrollment process to ensure prospective participants receive timely and effective nutrition services.
Collaborates with nutrition services staff and Center Director to conduct monthly and quarterly sanitation, safety and foodservice audits.
Participate in state and federal audit preparation activities.
Assist with planning and coordination of Dietetic Interns from affiliated RD and Dietetic Technician programs.
Baccalaureate or advanced degree from an accredited college with major studies in food and nutrition or related field. Must be a Registered Dietitian (RD) as confirmed by the Commission on Dietetic Registration.
Minimum of 1 year’s experience working with a frail or elderly population.
Three (3) years of experience working as a Registered Dietician in geriatric or managed care setting.
Excellent leadership, organization, and communication skills in settings with seniors, their families, and interdisciplinary team members.
Ability to translate best practices into culturally appropriate resources and work effectively with diverse populations.
Bilingual English/Mandarin/Cantonese preferred.
Current Basic Life Support (BLS) / First Aid Certification required

Completes safe transfers utilizing Durable Medical Equipment (DME) as required - lifts, gait belt and hoyers

Document participant progress, treatment plans, and outcomes accurately and in a timely manner using electronic medical records.
Monitor and reassess participant response to therapy interventions, modifying treatment plans as needed to optimize functional independence and quality of life.
Coordinate with external service providers, such as equipment vendors and community resources, to facilitate continuity of care and support participants' transition back to their homes.
Participate actively in IDT meetings, contributing insights and recommendations regarding participants’ occupational therapy needs and progress.
Adhere to center policies and procedures, maintain confidentiality of participant information, and comply with regulatory requirements.
Graduate of an accredited occupational therapy program; Master's degree in Occupational Therapy (OT) required.
Valid and active California Occupational Therapy license.
Minimum of 2 year of experience in occupational therapy, with a focus on geriatric or community-based care preferred.
Proficiency in Mandarin and/or Cantonese preferred but not required.
Strong clinical assessment and treatment planning skills.
Ability to work collaboratively in a multidisciplinary team environment and communicate effectively with participants, families, and colleagues.

Our mission is to empower seniors to age-in-place with dignity and provide their families peace of mind. We are building upon a proven Home and community based services model to create a culturally-competent and scalable PACE program. We are also building a comprehensive operating system focused on data and workflows that span across systems, processes, people, and care contexts. We want to empower our clinicians and staff with tools that deliver relevant data at the time and site of care and enable them to deliver exceptional care to our participants, which improve clinical outcomes, participant & provider satisfaction, and ultimately our strength as an organization.
We are a mission-driven, multidisciplinary team with deep healthcare, technology, and operations expertise, each inspired by our own personal stories of caring for seniors in our lives. Our name, Seen Health, was chosen to reflect our commitment to provide the highest standard of care to underserved older adults while respecting and incorporating their individual beliefs, heritage, and values, so that they can truly be seen.
