Who we hope to find:

PurposeMed is looking for a Patient Success Specialist to join our growing Patient Success team in supporting our HIV prevention and care business, Freddie. While our Support team excels at "fixing" immediate technical issues, the Patient Success team is our “stick with it” squad. Your mission is to proactively guide patients through the most friction-heavy parts of their Freddie journey—from completing their first consult to helping navigate lab testing and insurance, and ensuring they never miss a refill.

As a Patient Success Specialist, you are more than an executor; you are a problem-solver. You will manage outreach across both Canadian and US markets, navigating the nuances of different healthcare landscapes to ensure no patient falls through the cracks. You will use your "on-the-call" insights to help the Patient Success Lead refine our playbooks and you will play a key role in mentoring and ramping up future team members as we scale.

In this role, you’ll make an impact by:

Proactively Guiding & Retaining Patients

• Actively reaching out to patients to provide guidance on journey milestones, including using at-home testing kits, locating and booking in-person lab tests, and navigating the Freddie app
• Acting as the bridge between the patients and pharmacies to resolve prescription delays, overcome insurance hurdles (especially in the US market), solve delivery issues
• Moving beyond standard scripts to provide customized solutions for patients facing unique logistical or motivational barriers to care
• Identifying when a patient’s needs transition from logistical to clinical and ensuring seamless handoffs to our clinical team

Contributing to Operational Excellence & Insight Generation

• Following established Patient Success playbooks while proactively identifying gaps or areas where existing "scripts" aren’t meeting patient needs 
• Helping to develop new playbooks to overcome emerging barriers to care as we launch and scale our services
• Serving as the voice of the patient by synthesizing feedback from individual interactions and presenting actionable insights to improve our overall strategy
• Maintaining diligent records of patient interactions and outcomes to help the team track the impact of our success experiments

Being a Team Player

• Working cross-functionally with the Support and Pharmacy teams to ensure a unified patient experience
• Acting as a "buddy" and mentor for new hires, helping them ramp up on Freddie’s systems, culture, and the nuances of patient success

We need someone who has:

• 2+ years of experience in customer success, patient advocacy, or a high-touch support role
• A patient-first mindset with a natural ability to build trust quickly and demonstrate deep empathy when discussing sensitive health topics
• High agency and the ability to solve problems independently without waiting for a manual; comfortable making decisions to ensure patient success
• An outcome-oriented approach, with a focus on guiding patients through their entire journey and driving long-term retention
• Strong communication skills, both written (SMS/email) and verbal (phone), with a knack for simplifying complex processes for patients

• Contributing to peer-reviewed publications, white papers, and conference presentations in collaboration with Dr. Somayaji
• Collaborating with government agencies, non-profits, and community partners, including HIV CTN-affiliated organizations, on joint research initiatives
• Receiving structured mentorship in business analytics, product decision-making, and applied healthcare data science

We need someone who has:

• A PhD in Biostatistics, Statistics, Epidemiology, or a closely related quantitative field (conferred or imminent, PhD candidates with a confirmed defense timeline are welcome to apply)
• Demonstrated experience with advanced statistical analyses: regression modeling (linear, logistic, mixed-effects), ROC curves, risk score development and validation, survival analysis (Kaplan-Meier, Cox proportional hazards)
• Proficiency in R, including tidyverse and statistical modeling packages
• Working knowledge of SQL for querying healthcare data warehouses
• Excellent written and verbal communication skills and is able to present findings to clinical, operational, and non-technical audiences
• A proven ability to work independently and collaboratively in multidisciplinary teams

It would also be great if you have:

• Experience using Python (pandas, scikit-learn, statsmodels)
• Familiarity with cloud data warehouses (Snowflake, BigQuery, or Redshift) and version control (Git)
• Prior experience with real-world healthcare or clinical research datasets, particularly those with missingness, selection bias, or irregular observation times
• Exposure to BI tools (Looker, Tableau, Power BI) or R dashboard frameworks (Shiny, flexdashboard)
• Awareness of dbt or similar data transformation frameworks
• An interest in or prior experience with LGBTQ+ health, HIV/sexual health, gender-affirming care, or chronic disease management
• A prior connection to the HIV CTN, Alberta Innovates, University of Calgary research networks, or Dr. Somayaji's research group

Compensation:

• $80,000 CAD a year

We are looking for Physicians who are passionate about providing high quality gender-affirming care to our Foria Clinic patients. Foria clinic is primarily a virtual clinic providing trans, non-binary, and gender-diverse Canadians with gender-affirming healthcare. Reporting to the Medical Director of Foria, our team of physicians provide hormone assessments and prescriptions, surgical assessments and referrals, and general health care to trans and nonbinary people in Alberta. In this role, you’ll make an impact by:

• Working with patients from across Alberta who are highly motivated and excited to access care
• Supporting peoples’ gender journeys through hormone prescribing and referrals to surgeons
• Working collaboratively with a team including other Physicians, Nurse Practitioners and a Psychotherapist to ensure high quality care is provided to all patients
• Reducing wait-times for trans and nonbinary folks in all regions of Alberta
• Advocating for system changes to better support trans and nonbinary people

We need someone who has:

• A College of Physicians and Surgeons of Alberta (CPSA) license in good standing
• Canadian Medical Protective Association (CMPA) coverage for practice in Alberta
• An interest in gender-affirming care

It would also be great if you have:

• Previous experience and/or training in trans and nonbinary health care (e.g. from Trans Care BC, Rainbow Health Ontario, WPATH, etc.)

• Fee-for-service AH billing (psychotherapy and consult codes) managed by our operations team

Who we hope to find:

PurposeMed is looking for a Senior Clinical Programs Manager who can operate at the intersection of clinical credibility and operational rigor. This is a critical role for a strategic executor who thrives on taking complex pilots from concept to scale and ensuring that clinical initiatives are not just launched, but are deeply embedded into the fabric of our organization.

Reporting into the Director, Clinical Programs, you will drive the optimization and sustainability of our service offerings, directly influencing our long term organizational impact and the quality of care we provide to our patients. To truly understand and support our clinical programs, you will also have the opportunity to complete patient-facing work; this also ensures you can maintain the active practice hours required for your NP license.

In this role, you’ll make an impact by:

Driving Program Execution and Scaling Clinical Initiatives Strategically

• Leading the rollout of new clinical initiatives while ensuring every program remains perfectly aligned with our overarching organizational strategy
• Managing complex program lifecycles from initial concept through to full-scale operations with clear accountability for all milestones, including inheriting programs that are in-flight, incomplete, or underperforming and bringing structure, clarity, and momentum to them quickly
• Designing clinical pathways, workflows, and care models that are grounded in evidence and built to scale
• Bringing clinical teams through meaningful workflow changes in a way that results in genuine adoption, not just compliance
• Acting as the central liaison between Clinical Programs, Operations, and Pharmacy to ensure seamless execution

Building Operational Excellence and Long-Term Sustainability

• Developing transition frameworks including SOPs, workflows, and training materials to ensure programs are sustainable for the long term
• Designing and delivering optimizations for existing clinical programs to enhance efficiency and scalability
• Identifying inefficiencies within current clinical programs and implementing process improvements to reduce friction for clinicians and patients
• Driving the adoption of optimized processes across diverse cross-functional teams to ensure high quality outcomes

Leading With Data to Drive Performance and Inform Strategy

• Developing, tracking, and analyzing program KPIs to identify performance trends, risks, and new growth opportunities
• Translating data insights into actionable recommendations that inform leadership decision making and program scaling
• Preparing reports and briefings for senior leadership to communicate program impact and optimization success
• Leveraging clinical experience and evidence, as well as internal data, to act as a subject matter expert for cross-functional teams and initiatives
• Supporting senior leadership in maintaining strategic relationships with vendors, community partners, and external stakeholders to support patient outcomes and clinical programs

We need someone who has:

• A Master’s degree in Nursing or equivalent with at least one active Advanced Practice Nursing (NP) License within Canada 
• 5+ years of clinical experience and a proven track record in clinical program management, clinical operations, or healthcare strategy.
• Proven expertise in leading complex, multi-stakeholder initiatives from from pilot to full-scale sustainability
• Advanced skills in project management including the use of trackers, briefs, and risk mitigation frameworks
• Exceptional communication skills and the ability to build alignment across diverse internal and external groups
• A deep process orientation with experience writing SOPs and designing clinical workflows
• Experience working directly with or reporting to senior and executive leadership
• Familiarity with clinical regulatory requirements and compliance standards

It would also be great if you have:

• Experience in a high growth health tech or pharmacy operations environment
• Certification in Project Management or Lean Six Sigma
• Strong analytical capabilities with the ability to synthesize data into clear narratives for leadership

Compensation:

• $140,000 - $160,000 CAD a year
