Location & Schedule: Remote (United States). • Leading comprehensive initial assessments for those seeking preventative or HIV care, including but not limited to: history taking, risk assessment, and psychosocial evaluations

Supporting Seamless Asynchronous Patient Care (hours vary daily based on task availability)

• Providing asynchronous care to ensure timely and continuous support for patients, including ordering labs and diagnostics, interpreting results, prescribing medication, and clinical follow-up
• Maintaining accurate, detailed patient records in our Electronic Medical Record (EMR) system, using a wide variety of virtual tools to chart, prescribe, and connect with patients 
• Collaborating with and acting as a resource for our team of Registered Nurses and support team members, helping to support end-to-end patient care

We need someone who has:

• A Master's degree in Nursing and Primary Health Care Nurse Practitioner diploma
• Active Nurse Practitioner licenses in at least 10 states
• 2+ years of experience in primary care
• Experience providing virtual healthcare
• Excellent communication skills and empathetic care delivery
• Experience working in HIV Prevention and/or with the LGBTQ2AI+ community
• Daily availability for asynchronous patient care tasks (lab reviews, prescribing, follow-ups), plus 15 hours weekly for scheduled shifts, including live consultations. Task volume may vary based on state licensure and patient needs

Compensation:

While our Support team excels at "fixing" immediate technical issues, the Patient Success team is our “stick with it” squad. Your mission is to proactively guide patients through the most friction-heavy parts of their Freddie journey—from completing their first consult to helping navigate lab testing and insurance, and ensuring they never miss a refill.

As a Patient Success Specialist, you are more than an executor; you are a problem-solver. You will manage outreach across both Canadian and US markets, navigating the nuances of different healthcare landscapes to ensure no patient falls through the cracks. You will use your "on-the-call" insights to help the Patient Success Lead refine our playbooks and you will play a key role in mentoring and ramping up future team members as we scale.

Proactively Guiding & Retaining Patients

• Actively reaching out to patients to provide guidance on journey milestones, including using at-home testing kits, locating and booking in-person lab tests, and navigating the Freddie app

With a Partnerships Success Manager reporting to you to handle invoicing, data QA, and report preparation and a Senior Manager of Community to support on partner engagement, your job is to lead the relationships, design the systems, and drive the cross-functional work that makes our partners successful. Cultivating Strategic Partner Relationships (50%)

• Owning all covered entity partner relationships end-to-end, from onboarding through renewal and expansion
• Defining what an 11/10 experience looks like for our partners, which includes everything from running structured MBRs/QBRs to building partnership roadmaps
• Identifying and closing expansion opportunities through consultative, partnership-oriented selling
• Serving as the voice of the partner internally by translating their needs to product, clinical, and operations teams

Driving Consultative Growth (30%)

• Strategically growing our relationship with existing CE partners by helping them access new services through Freddie that benefit patient care and our respective missions
• Developing and delivering compelling pitches grounded in data, outcomes, and mission alignment
• Representing Freddie at industry events and conferences to build upon Freddie’s reputation in the 340B and community health ecosystem.

Optimizing Operational Excellence (20%)

• Oversee the Partnerships Success Manager who collaborates with other teams to ensure partners are invoiced and data reports are created and QA’d. Accountable for accuracy and partner trust; not expected to do the manual work.
• Ensure all operational processes are documented, reliable, and improving over time. The goal: if any person in this function left tomorrow, someone else could pick up.

We need someone who has:

• 7+ years in partnership management, strategic account management, or client leadership—with a track record of independently managing 10+ high-value relationships
• Demonstrated ability to build trust and credibility with mission-driven organizations (non-profits, community health, government-funded entities, or similar)
• Strong business development instinct—you spot growth opportunities and position them as value-add, not upsell
• Executive-level communication skills—you hold your own with clinic CEOs, CMOs, and board members without needing coaching or hand-holding.

This 1–2 year fellowship is designed explicitly as a transition-to-industry opportunity. • Demonstrated experience with advanced statistical analyses: regression modeling (linear, logistic, mixed-effects), ROC curves, risk score development and validation, survival analysis (Kaplan-Meier, Cox proportional hazards)
• Proficiency in R, including tidyverse and statistical modeling packages
• Working knowledge of SQL for querying healthcare data warehouses
• Excellent written and verbal communication skills and is able to present findings to clinical, operational, and non-technical audiences
