Key Responsibilities
The Hospice Nurse Preceptor is a trained professional nurse who administers skilled nursing care to patients and families who are terminally ill and meet hospice admission criteria in the home or facility setting. May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice. Responsible for overall coordination of the care delivered to assigned patient caseload. This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team, Medical Director, identifying patient, family, caregiver needs and providing for those needs in accordance with the Attending Physician’s orders and the IDG plan of care. This position is also responsible for the training of clinicians, including during the new hire orientation process and remedial training when required. May also perform facility education when requested.
