Job Summary
We are seeking an experienced Nephrologist (MD/DO) or New Graduate Nephrology Physician (MD) to provide comprehensive kidney care for residents in skilled nursing and long-term care facilities, as well as patients receiving dialysis treatment. This position is based out of our Hollywood, FL office and will serve as a collaborating physician for advanced practice providers while also providing telemedicine (TeleDoc) support across our network of facilities.
Responsibilities
- Provide comprehensive nephrology consultations and ongoing management for patients with acute and chronic kidney disease.
- Oversee dialysis treatment plans, adequacy, and patient outcomes for both facility-based and outpatient dialysis programs.
- Supervise and collaborate with Nurse Practitioners, Physician Assistants, and dialysis staff to ensure optimal clinical performance.
- Interpret and act on diagnostic tests, laboratory results, and imaging to guide individualized treatment plans.
- Manage associated conditions such as hypertension, anemia, and fluid/electrolyte disorders.
- Serve as the collaborating physician for Nurse Practitioners and Physician Assistants in accordance with state practice regulations.
- Participate in interdisciplinary care meetings and provide leadership for quality and performance initiatives.
- Provide telemedicine (TeleDoc) support and clinical oversight to providers across affiliated facilities.
- Educate patients and families on kidney disease management, dialysis options, and lifestyle modifications.
- Maintain accurate and timely clinical documentation and participate in ongoing care coordination.
- Operate primarily out of the Hollywood, FL office while supporting remote clinical teams and patients as needed.
- Clinical management: Perform comprehensive assessments, diagnose conditions, and adjust dialysis treatment plans and medications in collaboration with a nephrologist.
- Patient monitoring: Continually oversee patients' health during and after dialysis, interpret lab results, and respond to any complications.
- Interdisciplinary coordination: Lead and collaborate with the facility's nursing staff, dialysis technicians, and other healthcare providers to ensure cohesive patient care.
- Patient and family education: Educate patients and their families on kidney disease, the dialysis process, and critical lifestyle modifications like diet and fluid intake.
- Administrative documentation: Maintain precise and timely patient records, manage medication orders, and participate in quality improvement initiatives.
- Patient advocacy: Serve as the primary point of contact for patients and families, providing clinical and emotional support while coordinating care with specialists.
Requirements
- Must be an MD or DO from an accredited medical school.
- Must have completed an Internal Medicine Residency.
- Must have completed an accredited Nephrology Fellowship Program.

- Experience as a Medical Director of a dialysis center required.

Job Summary
We are seeking a Psychiatric Consultant to support our growing Collaborative Care Model (CoCM) program. This role provides psychiatric expertise through case review and consultation in partnership with Behavioral Health Care Managers (BHCMs) and Primary Care Providers (PCPs). This is a non-patient-facing role focused on population-based psychiatric consultation, clinical guidance, and collaborative treatment planning. All services are delivered in compliance with CMS CoCM guidelines.
Responsibilities
- Participate in scheduled psychiatric case reviews with BHCMs (typically biweekly or as clinically needed)
- Review BHCM-prepared case summaries, including:
- Diagnoses and relevant medical/psychiatric history
- Current medications and recent changes
- Behavioral health screening scores (e.g., PHQ‑9, GAD‑7, MOCA, BIMS)
- Clinical progress, concerns, or safety considerations
- Provide evidence-based psychiatric recommendations, including:
- Diagnostic clarification
- Medication recommendations
- Behavioral and environmental strategies appropriate for SNF/LTC settings
- Risk and safety considerations
- Collaborate with BHCMs to ensure recommendations are clearly understood and ready for PCP review
- Document recommendations in a HIPAA-compliant registry or EHR
- Electronically sign consult documentation in accordance with CMS requirements

We are seeking a qualified and enthusiastic Laser Phototherapy Technician to assist our dermatology division in providing laser phototherapy treatments to patients across different healthcare facilities. The successful candidate will be responsible for operating and maintaining laser equipment, preparing patients for treatment, and monitoring patients during and after treatment, while adhering to strict safety protocols.
Responsibilities
- Conducting initial patient consultations to determine their suitability for laser phototherapy treatment at different healthcare facilities
- Preparing patients for treatment, including the application of numbing creams and protective eyewear
- Operating laser equipment to deliver treatment to patients as directed by the dermatologist, while adhering to strict safety protocols

Requirements
- Minimum of a high school diploma or equivalent

As a CCM Manager, you will be responsible for overseeing the daily operations and performance of MedElite’s Chronic Care Management (CCM) and Principal Care Management (PCM) programs. Key Responsibilities
Program Leadership & Daily Operations
- Oversee daily operational performance of CCM/PCM programs, ensuring adherence to CMS requirements and organizational standards
- Monitor and verify team attendance daily (via EMPEON and in-person checks)
- Serve as the primary point of contact for coordinator questions, workload issues, and operational escalations
- Implement and optimize workflows; drive continuous improvement in collaboration with Clinical Directors and Operations leadership
- Attend and present in required meetings or ensure appropriate coverage
Staff Oversight, Support & Performance Management
- Supervise, coach, and provide day-to-day direction to CCM/PCM clinical coordinators
- Deliver targeted coaching and support for low-performing team members
- Track productivity, accuracy, and compliance metrics; manage KPIs and performance improvement plans
- Execute progressive discipline in partnership with HR and leadership
- Foster a positive, engaged, and accountable team culture to support retention
Onboarding, Training & Competency
- Conduct interviews and lead onboarding and orientation for new hires, including a structured 30–90 day training period
- Train staff on CMS documentation and billing requirements, coding standards, care plan development, and internal systems (ROPHE, EMSOW, eMARS)
- Develop and maintain SOPs, job aids, and training materials

Quality Assurance & Chart Oversight
- Perform routine chart reviews, audits, and documentation checks to ensure CMS compliance and internal quality standards
- Provide corrective feedback, retraining, and follow-up coaching based on findings
- Identify trends and systemic issues to improve overall program quality and reduce rework
- Support charting and documentation corrections during high-volume periods
Administrative & Systems Management
- Approve or deny PTO requests and resolve payroll discrepancies or missed punches
- Manage employee system access across required platforms (ROPHE, EMSOW, eMARS)
- Maintain and report monthly retention data, including hires, terminations, and attrition
Communication & Collaboration
- Lead daily team huddles to align on priorities, goals, and performance expectations
- Conduct weekly 1:1 check-ins and coaching sessions
- Partner cross-functionally with BHI/CoCM, Population Health, Quality Assurance, and Operations teams
Requirements
- Bachelor’s degree required; healthcare-related field preferred
- 3+ years of experience in healthcare operations, care management, or clinical program leadership
- Prior experience managing teams in CCM, PCM, or population health programs preferred
- Strong understanding of CMS guidelines, documentation, and billing practices
- Proven ability to manage performance, analyze data, and drive operational improvements
- Excellent communication, leadership, and organizational skills

Key Responsibilities:
Consent Coordinator

Place daily outbound calls to patients' families, NOKs, or responsible parties to explain the purpose, benefits, and structure of applicable Medicare programs.

Clearly articulate the goals, eligibility, and patient benefits of the programs in a professional and compassionate manner.

Obtain and document verbal or written consent in accordance with CMS and company protocols.

Accurately log all call outcomes and maintain meticulous documentation in designated platforms.

Meet or exceed a conversion rate goal of ≥60% of all successfully connected calls (i.e., where the NOK is reached).

Participate in pre-call preparation and post-call documentation for audit readiness.

Job Summary
We are looking for a dependable and detail-oriented Housekeeper to ensure our Florida offices remain clean, organized, and welcoming on a daily basis. The Housekeeper will be responsible for maintaining cleanliness in offices, restrooms, kitchens, and common areas through routine and deep-cleaning tasks. This role requires someone with prior cleaning experience, strong attention to detail, and the ability to work independently while following established cleaning standards. Responsibilities
Daily Cleaning Tasks:
- Remove all garbage and dispose of waste in designated containers.

- Wipe down desks, countertops, tables, and high-touch surfaces.
- Dust furniture, fixtures, and computer equipment.
