Position Description

Bluebird Kids Health is seeking a dedicated Outreach and Engagement Specialist to proactively connect with the children and caregivers that we serve, fostering meaningful engagement in our care model. Reporting to the CARES Center Operations Manager, this role is a valuable first point of contact for new patients, and pivotal in enhancing engagement and driving improved health outcomes. This individual will implement data-driven strategies to conduct outreach activities aimed to address care gaps, improve engagement, onboard new patients, and support reengagement of established patients. As a member of the CARES Center team, the Outreach and Engagement Specialist facilitates access to Bluebird Kid’s Health services and timely coordination of care with the longitudinal care team.

What You’ll Do

Prospective Member Outreach

• Conduct proactive outreach to prospective families to encourage them to join Bluebird Kids Health.
• Engage with leads generated through marketing campaigns, referrals, and community events, providing information on our services and guiding them through the enrollment process.
• Collaborate with the marketing team to ensure consistent messaging and promotion of Bluebird Kids Health to prospective patients.

Established Patient Outreach

• Reach out to existing patients to encourage scheduling of well care visits, vaccinations, and other preventive care services.
• Follow up with patients regarding their plan of care, ensuring they attend recommended appointments, receive necessary follow-up care, and are connected to community resources as appropriate.
• Identify and engage patients who may have missed appointments or are overdue for follow-up care, using appropriate channels to reconnect with them.

Patient Engagement and Retention

• Develop and maintain strong relationships with both prospective and current patients, ensuring a positive experience with the practice.
• Provide exceptional customer service and patient education, addressing any concerns or questions patients may have about their care.
• Track and report on outreach efforts, outcomes, and key metrics related to patient acquisition and retention.
• Support patients in navigating Medicaid eligibility and redetermination processes. Provide education and referrals as needed to facilitate access to plan sponsored benefits and services.

Collaboration and Coordination

• Work closely with other members of the CARES Center team to ensure smooth patient onboarding and care coordination.
• Collaborate with clinical teams to understand patient needs and ensure that outreach efforts align with care plans and clinical priorities.
• Assist in identifying barriers to care and develop strategies to address them, improving patient engagement and health outcomes.

What You’ll Need

• High School diploma or equivalent
• 1-2 years experience in healthcare contact center
• Available to work Monday-Friday 8:00a-5:00p ET
• Willing to work some evening and weekend shifts
• Strong communication and interpersonal skills, with the ability to effectively build rapport with the children and caregivers that we serve
• Excellent organization and time-management skills to effectively manage multiple tasks simultaneously
• Proficiency in using multiple technology platforms including but not limited to, electronic health records (EHRs), Customer Relationship Management (CRM) platforms, digital patient engagement tools and productivity tools
• Bilingual proficiency in English and Spanish is strongly preferred
• Experience with pediatric populations is strongly preferred

Position Description 

Bluebird Kids Health is seeking a dynamic Care Coordinator for our CARES Center (Center for Access to Resources, Education, and Support). The ideal candidate is a self-starter with strong interpersonal skills, experience in a medical receptionist role, and a proven ability to deliver results in a fast-paced, growth-oriented environment. 

What You’ll Do  

• Serve as a key administrative point of contact for care coordination needs, helping families and the care team stay aligned on the next steps. 
• Manage the provider inbox by sorting clinical documents, administrative messages, phone messages, and unlabeled/unknown documents; route items appropriately and track to completion. 
• Manage Inbound and Outbound calls, and send messages to schedule, confirm, and remind families about follow-up visits (e.g., well child check-ups, ADHD follow-ups, behavioral consults) and to address overdue items. 
• Track ongoing needs for patients who require more frequent follow-up (e.g., chronic conditions) by maintaining task lists, reminders, and documentation that support timely check-ins. 
• Coordinate outside appointments and diagnostics (MRI, CT, ultrasound, X-ray), including gathering required paperwork, supporting prior authorizations, and ensuring results are received and routed. 
• Complete referrals and prior authorizations as required by insurance. 
• Support families seeking mental/behavioral health services by collecting information, coordinating next steps, and sharing available options and resources. 
• Mange prescription refill requests according to clinic workflows and ensure items are routed for appropriate review and documented in the EMR. 
• Maintain patient records by scanning, uploading, and organizing documents in the chart. 
• Verify and update patient demographics and key information during each interaction to support accurate communication and records. 
• Document interactions and coordination activities clearly and consistently in the EMR, following HIPAA and confidentiality standards. 
• Prepare 504 letters and related forms as needed, following established templates and internal processes. 
• Maintain an up-to-date directory of specialty and community resources (including contact information) to support efficient scheduling and handoffs. 
• Perform other duties as assigned.

Position Description

Bluebird Kids Health is seeking a dedicated Patient Access Specialist to provide seamless access to care for the children and families we serve, playing a key role in fostering meaningful engagement with our care model. As the first point of contact, this role is essential in delivering a positive patient experience by promptly answering inquiries, scheduling appointments, and ensuring timely resolution of patient needs. Reporting to the CARES Center Operations Manager, this role is vital in connecting patients to the services they need and enhancing overall engagement.

In this role, you will actively manage patient interactions, utilizing our systems to coordinate care, manage appointments, and address any concerns. Your efforts will help ensure timely and equitable access to services. As a valued member of the team, you will help drive improved health outcomes and positive patient experience.

What You’ll Do

Omnichannel Engagement: Inbound and Outbound

• Manage inbound patient interactions across multiple channels, including phone, SMS, patient portal messages, and webform inquiries.
• Proactively initiate outbound engagement to follow up on inquiries or messages, ensuring continued patient support and timely resolution of concerns.

Appointment and Schedule Management

• Coordinate and manage patient appointments, ensuring timely scheduling and

rescheduling in alignment with operational standards and patient needs.

• Optimize appointment availability, working closely with clinical teams to address

scheduling preferences and potential conflicts.

• Track and follow up on missed or rescheduled appointments to ensure patients

receive timely care and minimize gaps in the care process.

Documentation Compliance

• Ensure accurate and complete documentation of patient demographics, insurance,

and appointment details in the EMR system, following internal standards.

• Verify and update patient information during each interaction, maintaining data

integrity and consistency across all systems.

• Always uphold HIPAA compliance and patient confidentiality while maintaining

thorough and accurate records of all interactions.

Collaboration & Communication:

• Coordinate with the interdisciplinary care team to align patient care, scheduling

preferences, and resolve any issues.

• Facilitate clear communication between healthcare providers, support staff, and patients to ensure smooth care delivery.
