Position Description
Bluebird Kids Health is seeking a dynamic Care Coordinator for our CARES Center (Center for Access to Resources, Education, and Support). The ideal candidate is a self-starter with strong interpersonal skills, experience in a medical receptionist role, and a proven ability to deliver results in a fast-paced, growth-oriented environment.
What You’ll Do
- Serve as a key administrative point of contact for care coordination needs, helping families and the care team stay aligned on the next steps.
- Manage the provider inbox by sorting clinical documents, administrative messages, phone messages, and unlabeled/unknown documents; route items appropriately and track to completion.
- Manage Inbound and Outbound calls, and send messages to schedule, confirm, and remind families about follow-up visits (e.g., well child check-ups, ADHD follow-ups, behavioral consults) and to address overdue items.
- Track ongoing needs for patients who require more frequent follow-up (e.g., chronic conditions) by maintaining task lists, reminders, and documentation that support timely check-ins.
- Coordinate outside appointments and diagnostics (MRI, CT, ultrasound, X-ray), including gathering required paperwork, supporting prior authorizations, and ensuring results are received and routed.
- Complete referrals and prior authorizations as required by insurance.
- Support families seeking mental/behavioral health services by collecting information, coordinating next steps, and sharing available options and resources.
- Mange prescription refill requests according to clinic workflows and ensure items are routed for appropriate review and documented in the EMR.
- Maintain patient records by scanning, uploading, and organizing documents in the chart.
- Verify and update patient demographics and key information during each interaction to support accurate communication and records.
- Document interactions and coordination activities clearly and consistently in the EMR, following HIPAA and confidentiality standards.
- Prepare 504 letters and related forms as needed, following established templates and internal processes.
- Maintain an up-to-date directory of specialty and community resources (including contact information) to support efficient scheduling and handoffs.
- Perform other duties as assigned.
