BAYADA Home Health Care is seeking to fill the position of Clinical Director for our Erie, PA Home Health office. This office provides best in class care to adult and geriatric clients throughout Columbus and surrounding counties.

As a Home Health Director your day may look like this!

Lead, inspire and motivate others to provide exceptional care.
Dynamic Clinician who understands how to maintain the highest levels of client care and customer service.
Demonstrate the ability to build relationships with employees, referral sources, and community resources while growing this Medicare Certified Home Health Office.
Responsible for the management and operations of the office, including business development, budgeting, planning, recruiting and fiscal management.
Monitor the quality and appropriateness of all services provided by your staff to ensure compliance and client satisfaction while ensuring adequate staff education, training and evaluation.
Supervise a team of field nurses and therapist
Conduct supervisory visits as needed within the homes.
Lead, inspire and motivate others to provide exceptional care.
Visit our adult and geriatric clients for supervisory/assessment visits in the Erie, PA area.
Facilitate new hire orientation and training the new nurses.
Coordinate and/or deliver in-services for field staff.
2 Years of Oasis experience preferred.

BAYADA Home Health Care is looking for a compassionate and dedicated Full Time Licensed Practical Nurse (LPN) to join our team. This office services our adult clients on a per visit basis in homes throughout Erie County.

One year prior clinical experience as a licensed Practical Nurse is required.

As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

Making home visits to clients living in designated territories.
Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
Case management and coordination.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

A current license as a Licensed Practice Nurse in the state of PA.
A minimum of one year of recent, verifiable experience as an LPN.
Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
Prior home care experience strongly preferred, but not required.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.
