Registered Nurse (RN) Responsibilities:

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist, SLP, for a per diem / part time opportunity to complete home health visits with adult and geriatric patients on a per visit basis.

*Full Time opportunity available if candidate is open to covering a larger territory, which includes travel between Longmont - Castle Rock, CO areas.

At least one year of experience as an Speech Language Pathologist is required; prior home health experience as an SLP is a plus. As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Speech Language Pathologist, SLP Responsibilities:

Make home visits to clients in designated geographic territories.
Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.
Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.

Speech Language Pathologist, SLP Qualifications:

A current license as a Speech Language Pathologist in Colorado.
Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;
Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification, or;
Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Duties of a Clinical Manager include:

Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals.
Processing incoming referrals.
Management and mentoring of Clinical Associates and field staff

Documentation review, including OASIS and Home Care Home Base (HCHB)
Monitoring quality assurance.

Qualifications:

An active Colorado or Compact Registered Nurse license.
Home Health experience preferred.
Recent Medicare Certified Home Health Care experience.

HCHB a plus.
Strong people management and leadership experience highly desired.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a per diem weekend opportunity performing home health visits for our Denver North Visits office, located in Broomfield, CO. This team works with adult and geriatric clients on a per visit basis.

This is a weekend position - candidate will be asked to complete at least four (4) Start of Care visits per weekend.

$5,000 Incentive if candidate is able to commit to 4 SOC per weekend (2 Saturday + 2 Sunday) OR
$5,000 Incentive if candidate is able to commit to 20 productivity points per weekend (combination of Fri, Sat, Sun, and/or Mon) - benefits eligible!

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

A current CO Physical Therapist - PT license.
A minimum of one year of recent work experience as a Physical Therapist - PT.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
