POSITION SUMMARY

The primary role of the Unit Supervisor is to manage the Friedman Unit(s) assigned, supporting the Clinical Unit Manager and Director of LTC/Post-Acute Care, and plan, organize, develop, and implement nursing care strategies for the delivery of efficient and effective care. Unit Supervisor ensures care provided is consistent with local, state, and federal guidelines and regulations. This position is responsible for ensuring that high quality care is delivered to all residents/patients of the facility.

KEY RESPONSIBILITIES 

  • Make rounds at the beginning of the shift to establish a baseline for assessment of resident needs and supervision of care.
  • Discuss with the attending MD regarding any changes in the resident’s condition, medication, and treatment. Check the attending MD’s binder and ensure that the MD addresses all problems and signs papers that need a signature.
  • Oversee the provision of appropriate medications, treatments, and general nursing services according to the resident’s/patient’s care plan and physician-directed orders.
  • Manage hospital discharge orders to initiate the admission process. Responsible for admission orders and helps in transcription of MARs and TARs.
  • Plan and evaluate programs and recommend changes as needed.
  • Provide a professional role model.
  • Provide support in the development, implementation, and goals of the assigned unit(s).
  • Monitor and evaluate unit-specific Quality Assurance and Improvement Activities.
  • Identify opportunities for improvement. Initiate a Performance Improvement Plan (PIP) as necessary.
  • Work in collaboration with other members of the multidisciplinary team to ensure an integrated approach.
  • Ensure compliance with local, state, and federal guidelines and regulations.
  • Prepare the unit(s) for the annual California Department of Public Health (CDPH) survey, resulting in no preventable significant citations from CDPH. When needed, help create Plan of Correction (POCs), Informal Dispute Resolutions (IDRs), etc.
  • Cover unit as needed related to staffing needs.
  • Monitor, counsel, and document staff performance, and implement corrective actions, including termination when necessary.
  • Hire staff, participate in orientation of new employees, and ongoing education of assigned staff.
  • Assume delegated responsibilities for the day-to-day management and supervision of the unit in the absence of the unit manager. Assume unit manager responsibilities as assigned.
  • Ensure adequate staffing levels allowing admissions without interruption, as well as the prevention of the facility from falling below required staffing ratios.
  • Monitor staff breaks, missed punches, overtime, and schedule each shift.
  • Monitor, counsel, and document staff performance.
  • Develop a strong and cohesive Nursing Team that practices strong inter-departmental collaboration, as well as acts as a role model and leads by example.
  • Collect, analyze, and report Quality of Care data and trends.
  • Serve on various committees including, but not limited to: Psychotropics, Falls, Safety, Skin, Weight, QAPI, RTA/Transfers, Infection Control, etc.
  • Conduct Clinical Investigations and ensure timely reporting of all unusual occurrences and adverse events.
  • Outreach to families regarding care concerns.
  • Conduct routine performance evaluation of staff.
  • Partner with other members of leadership to maintain a culture/environment that practices quality customer service and service recovery when needed
  • Participate in the orientation of new employees and ongoing education of assigned staff.
  • Proactively identify opportunities for improvement.
  • Perform other duties as assigned.

JOB DETAILS

  • Pay Rate: $141,440 - $145,600
  • Hours: 40 Hours per week
  • Shift Schedule: Monday through Friday

REQUIRED QUALIFICATIONS

  • Current California Registered Nurse (RN) licensure and BSN required
  • At least one year of clinical work experience in a post-acute and long-term care environment
  • Knowledge of State and Federal/CMS guidelines and regulations preferred
  • Able to demonstrate leadership ability and teaching skills
  • Able to provide direction and coordination for a multi-faceted program, including problem resolution within the facility.
  • Demonstrates problem-solving skills with the capacity to make quick and definitive decisions
  • Excellent organizational and interpersonal skills
  • Strong communication skills (verbal and written)
  • Self-motivated and able to work independently and work on multiple tasks in a deadline-oriented environment
  • Ability to work effectively in a deadline-oriented environment with multiple priorities.
  • Ability to be accurate, concise, and detail-oriented
  • Computer proficiency including word processing, ability to use email and Internet programs, familiar with computerized data entry
  • Ability to sit and/or work on a computer for long periods
  • Ability to communicate effectively with colleagues, residents and family members, and at all levels of the organization
  • Ability to maintain confidentiality

PHYSICAL REQUIREMENTS

  • Ability to lift a minimum of fifteen (15) pounds
  • Ability to exert maximum muscle force to lift, push, pull or carry
  • Ability to use abdominal and lower back muscles over time without fatigue
  • Ability to stand and/or sit for extended periods
  • Ability to bend, stoop, stretch, twist, sit, and reach
  • Ability to use voice/speak for prolonged periods of time
  • Fine motor skills; good visual and auditory acuity
