Physical Therapist (PT) – Part-Time
Kern River Transitional Care
Pay Range: $65.00–$70.00 per hour, based on experience

Kern River Transitional Care is seeking a compassionate and motivated Part-Time Physical Therapist to join our outstanding rehabilitation team. Essential Duties and Responsibilities
- Evaluate residents’ mobility, strength, balance, coordination, range of motion, and functional abilities.
- Develop individualized treatment plans based on each resident’s needs, goals, and medical condition.
- Provide skilled physical therapy services to improve mobility, independence, safety, and overall function.
- Help residents regain the ability to walk, transfer, maintain balance, and complete daily activities safely.
- Establish measurable goals and modify treatment plans based on residents’ progress and changing needs.
- Provide therapeutic exercise, gait training, balance activities, transfer training, and other appropriate interventions.
- Recommend and train residents in the safe use of walkers, wheelchairs, canes, and other adaptive equipment.
- Educate residents, families, caregivers, and facility staff regarding treatment plans, safety, positioning, and mobility techniques.
- Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and on time.
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
- Participate in resident care conferences, discharge planning, and quality-improvement activities.
- Supervise Physical Therapist Assistants and rehabilitation support staff as permitted by law and facility policy.
- Maintain residents’ privacy and the confidentiality of all health information.
- Follow all professional standards, facility policies, safety procedures, and regulatory requirements.
- Perform additional duties as assigned.
Qualifications
- Graduate of an accredited physical therapy program.
- Current and unrestricted California Physical Therapist license required.
- Current CPR/BLS certification required.
- Skilled nursing, rehabilitation, long-term-care, or geriatric experience preferred.
- New graduates are encouraged to apply.
- Strong clinical assessment, communication, documentation, and time-management skills.
- Ability to work independently and collaboratively as part of an interdisciplinary team.
- Knowledge of PointClickCare or another electronic medical record system preferred.
