Job Summary: 

The PACE Dietitian Manager is responsible for the medical nutrition program for PACE participants.

The PACE Dietitian Manager is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing nutrition assessments for new PACE participants and nutrition reassessments on existing PACE participants as part of the PACE IDT care planning process.

The PACE Dietitian Manager is responsible for assessing participants' nutritional and dietary needs and incorporating industry best practices to implement appropriate nutritional and dietary interventions for PACE participants in collaboration with the IDT.

The PACE Dietitian Manager is responsible for planning food services at the PACE Center and ensuring participant food preferences are considered in PACE Center menu planning.

The PACE Dietitian Manager is responsible for providing nutrition and diet education to participants, caregivers, and PACE staff.

Essential Job Functions

  • Participates as a PACE Interdisciplinary Team (IDT) member and is responsible for completing nutrition assessments for new PACE participants and nutrition reassessments on existing PACE participants as part of the PACE IDT care planning process.
  • In collaboration with the primary care provider and IDT, develops and implements nutrition and diet care plans for participants, identifying and considering the medical, physical, social, and emotional needs of PACE participants.
  • Assesses PACE participants’ food security status and recommends to the IDT measures to address participant food security issues through PACE service delivery interventions and the care planning process.
  • In concert with the primary care provider, assesses participants’ needs for nutritional supplements.
  • Monitors PACE participants for changes in their health or psychosocial status requiring a nutrition reassessment. Promptly communicates pertinent changes in participant nutritional or dietary status to the IDT.
  • Attends IDT meetings and morning meetings and reports on participants' nutritional status, including changes to individual participant food intake and notable weight changes. Based on clinical observations or nutrition reassessments, make recommendations to the IDT for incorporating or modifying therapeutic diets and recommending dietary modifications to participant care plans.
  • Responsible for daily food services planning at the PACE Center, ensuring participants’ dietary restrictions are enforced and participant food preferences considered.
  • Supervise, mentor, and evaluate nutrition staff; oversee staffing, scheduling, and performance to ensure continuity and accountability.
  • Completes and maintains documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy.
  • Serves as a nutrition and diet subject matter expert and communicates to the IDT best practices in nutrition management for a frail elderly population.
  • Responsible for providing nutrition education to participants and caregivers. Home visits may be necessary to provide participants and caregivers with nutrition education or nutritional assessments/reassessments.
  • Assists with meal and snack service in the PACE Center as needed.
  • Conducts sanitation and safety checks for kitchen and kitchen equipment.
  • Supports the PACE QI program and participates in PACE QI activities as assigned.
  • Minimum Requirements

  • Bachelor’s degree in Nutrition from an ADA-accredited institution.
  • Certified as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
  • Minimum 1-year work experience as a Dietitian.
  • Registered member of the ADA and registered by the Commission on Dietetic Registration.
  • Leadership and management experience.
  • Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
  • One year of experience working with a frail or elderly population, or in the absence of such knowledge, receive appropriate training on working with a frail or elderly population upon hiring.
  • Before working independently, meet a standardized set of competencies for the Dietitian position established by the PACE plan.
  • Ability to make home visits as appropriate and necessary.
  • Experience working in Electronic Health Records (EHRs).
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
  • Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
  • Have not been excluded from participating in Medicare and Medicaid programs.
  • Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
  • BLS certified.
  • ServSafe certified
  • Certifications, Licenses, Registrations

  • Certified as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
  • Registered member of the ADA and registered by the Commission on Dietetic Registration.
  • BLS certified.
  • ServSafe certified
