Responsibilities:
- Provides direct care to residents.
- Coordinate, supervise and evaluate resident care.
- Assist with safety assurance for residents and nursing staff.
- Communicate with nursing and other departments and service providers.
- Exhibit positive professional image to residents, visitors, and facility staff.
- Perform related tasks as required.

Qualifications:
- Education: Graduate of approved nursing program
- Licensure: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
- Experience: At least 6 months of direct care nursing practice desired; long term care experience preferred

Responsibilities
We are currently seeking an enthusiastic and professional Full Time Home Health LPN.
- This role is responsible for providing assistance with the primary caseload for our skilled home health care patients.
- This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, Ohio Home Health licensure laws and agency policies and procedures.
Qualifications
- Education: Graduate of approved nursing program
- Licensure/Certification: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
- Experience: A minimum of 2 years’ direct patient care experience in hospice or home care
- Since this is a community-based ministry, all LPN's are required to be licensed drivers with automobiles in good working conditions and have automobile insurance in accordance with Ohio laws, they must also have a clean driving record for insurability purposes.
- This position will serve the county where you reside.

- Ensures completion of and maintains documentation of fire safety evacuation plan, fire drills, disaster plan, smoking policy, fire alarm testing, fire extinguisher maintenance and testing reports, generator testing logs, water temperatures, flame spread ratings of interior finishes, and other maintenance records.
- Performs any other duties as assigned.
QUALIFICATIONS:
- Education: High School Diploma or equivalent is required
- Licensure/Certification: Holds a valid Ohio driver’s license
- Experience: General Maintenance - Electrical, Plumbing and Carpentry Experience, Mechanical Experience on H.V.A.C., Mowers and Vehicles.

Overview
The Business Office Coordinator is responsible for the daily business office needs including payroll, Resident Trust Fund, and other accounting functions.
Responsibilities
- Process bi-weekly payroll.
- Provide new hire orientation including paperwork, fingerprinting, back ground check, licensure verification, and reference checks.
- Maintain and audit employee files.
- Maintain worker’s compensation system, background check log and OSHA log.
- Complete new hire and termination processes.
- Provide support and follow through with unemployment issues.
- Collect and maintain elder assistant schedules for state documentation purposes.
- Code invoices and billing; reconcile benefits bills
- Ensure applications and marketing materials are easily available.
- Reconcile petty cash and resident funds monthly.
- Maintain attendance and tardy system documentation
- Ensure postage is readily available.
- Maintain and order office supplies.
- Maintain the Medical Supply Ordering System including new hire entry, monthly billing, and non-clinical product entry.
- Be the corporate business/support liaison.
- Performs any other duties as assigned
Qualifications
- Education: Prefer post-secondary education with marketing experience or human resources experience.
- Licensure/Certification: Notary Public, preferred, not required
- Experience: 1-5 years related work experience. Payroll experience strongly preferred ideally with Ulti Pro software, but not required
- Computer Skills: Word, Excel, UltiPro, but not required.

Responsibilities
- Provides personal care
- With respect to patient rights, ensures all care is provided with respect and dignity to patients and their families
- Demonstrates and maintains a positive, professional manner
- Keeps a record of hospice STNA plan of care for each patient assigned
- Observes, records, and reports the patient’s care, symptoms, emotional and physical status
- Watches, observes, and reports any physical and gross behavioral changes in the patient’s condition or family situation to the RN
- Assists in feeding the patient as needed
- Communicates appropriately with the patient/family and agency personnel
- Attends required in-services and meetings as identified by the agency (must have twelve (12) hours of in-services a year).
- Attends and participates in regularly scheduled Hospice interdisciplinary team (IDT) meetings to coordinate plan of care, follow up on changes and other communication, problem-solve, receive staff support and in-service education
- Participates in Quality Assurance/Performance Improvement (QAPI) activities, as indicated
- Will need to travel to see patients between Lebanon, Middletown, and Springboro
Qualifications
- Minimum age requirement of 18.
- Education: Must have completed the 9th grade, with ability to read or write at an adequate level. High school graduates and GED preferred.
- Certification: CNA/State Tested Nursing Assistant (STNA) certification - Must be on the State Tested Registry in Ohio and remain active, CPR required.
- Experience:
- Minimum of one (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
- Must demonstrate the ability to be able to work within a multidisciplinary team setting.
- Understanding of Hospice philosophy and needs of terminally ill.
- Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements.
