Liquid Personnel is recruiting Social Workers to join its client’s Adult Hospital Discharge team based in Manchester.

What will your responsibilities be?

• Carry out Care Act assessments, strengths-based and person-centred, for citizens recently discharged from hospital.
• Complete timely reviews of hospital-arranged care packages, assessing for long-term support before transfer to community teams.
• Conduct Mental Capacity assessments, Best Interest decisions, and safeguarding work where required.
• Work independently in a hybrid model, seeking guidance for complex cases, with no duty function

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
