Licensed Practical Nurse (LPN) – Home Health Visits

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team.
In this role, you'll provide one-on-one skilled nursing visits to adult clients throughout Hudson County, NJ, helping them safely remain in the place they call home. As a valued member of our interdisciplinary care team, you'll deliver high-quality clinical care while building meaningful relationships with your clients.
A minimum of one year of clinical experience as a Licensed Practical Nurse is required.
Responsibilities
- Provide skilled nursing visits to clients throughout designated territories.
- Administer medications, wound care, treatments, and other physician-ordered nursing interventions.
- Monitor client conditions and communicate changes to the Clinical Manager and Client Services Manager.
- Complete comprehensive nursing assessments as needed.
- Execute and document physician orders accurately and in a timely manner.
- Coordinate care with physicians and the interdisciplinary care team.
- Maintain accurate documentation of client assessments, interventions, and outcomes using BAYADA's electronic medical record (EMR) system.
Qualifications
- Current New Jersey Licensed Practical Nurse (LPN) license.
- Minimum of one year of recent, verifiable clinical experience as an LPN.
- Graduate of an accredited nursing program.
- Home health experience strongly preferred, but not required.
- Ability to work independently and manage time effectively.
- Strong communication, organizational, and interpersonal skills.
- Experience with electronic medical records (EMR) preferred.
- Reliable transportation and ability to travel throughout the assigned territory.

Additional Information:
Base Pay, depending upon qualifications, status, and subject to negotiation:
$46.00 - $52.00 per point
