Central Gardens Post Acute is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house rehabilitation team. In-House Therapy Program
Work directly with our facility-based rehabilitation team and focus on quality resident care, clinical outcomes, and continuity of treatment.
Collaborative Team Culture
Work alongside nursing, therapy, dietary, social services, activities, and other healthcare professionals who are committed to providing excellent resident care.
Responsibilities
Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
Provide skilled physical therapy services based on residents’ needs and goals.
Implement evidence-based interventions to improve strength, mobility, balance, endurance, and functional independence.
Monitor resident progress and adjust treatment plans as appropriate.
Educate residents and families regarding therapy goals, exercises, mobility, and safety.
Collaborate with PTAs and the interdisciplinary care team to support positive clinical outcomes.
Maintain accurate and timely clinical documentation.
Promote resident safety, dignity, independence, and quality of life.
Participate in care planning and interdisciplinary team activities as needed.
Qualifications
Current California Physical Therapist (PT) License in good standing required.
Strong communication and interpersonal skills.
Compassionate, resident-centered approach to care.
Ability to work effectively as part of an interdisciplinary team.
Strong clinical judgment and organizational skills.
