Position Summary:

The Student Case Manager predominantly works with complex student situations that have impacted the student’s ability to successfully function in the University community, or when the student has been disruptive to the community and requires significant behavioral changes to remain in or return to the University. The case manager will help facilitate a student’s access to, and appropriate utilization of, University and/or community services.

The Student Case Manager reports directly to the Associate Director of Student Case Management and Referral Coordination. This person establishes and maintains very close working relationships with University Housing, CSU Health Network, Student Disability Center, Collaborative for Student Achievement, Registrar, Office of Financial Aid, Student Resolution Center, Colorado State University Police, Parent and Family Programs and others. In addition, the Student Case Manager develops and maintains strong collaborative relationships with community and other campus agencies, including locals behavioral health facilities.

The Student Case Manager is a University administrative role. While the case manager may develop strong supportive relationships with students and staff, they do not engage in individual therapeutic relationships.

The Student Case Manager is currently designated as a permanent full-time Administrative Professional.

Description of the Work Unit:

The Student Case Management & Referral Coordination (SCM) office is a part of the Division of Student Affairs at Colorado State University and offers a full complement of services available to students to support their personal and academic success. In difficult situations, which might include mental health, personal or family crisis, Basic Needs insecurity, illness, or injury a student might benefit from a coordinated needs assessment, appropriate referral, and follow up as the student progresses through the situation. SCM focuses on minimizing the impact behaviors or negative situations on self, faculty and staff, other students, and their academic career.


As the CSU Student Case Management and Referral Coordination team we strive to be inclusive, transparent, compassionate and solution-focused. We believe in the resiliency, self-determination and strengths of our students and aim to address their needs in a holistic and creative manner.


Essential Job Duties:

  • Serve as a primary contact for students in crisis, with complex needs, or who are hospitalized.
  • Assist in student academic planning through collaborative relationships with Academic Advisers/Support Coordinators Collaborative for Student Achievement and/or departments and colleges. This could include assisting with student planning for catching up with missed academic work, adjusting an academic course load, facilitating a university withdrawal when necessary, and helping a student plan for a successful return to the academic environment.
  • Consult regularly with the various departments on campus regarding services, access, and care particularly the CSU Health Network, Student Disability Center, SLiCE, Title IX Programs, and Student Resolution Center.
  • Develop connections with staff in various university offices, including but not limited to the Collaborative for Student Achievement, CSU Police Department, CSU Health Network, Student Organizations, SLiCE, Athletics, Student Resolution Center, Support & Safety Assessment, Student Disability Center, etc. 
  • Counsel, consult, and advise students on an individual basis as necessary, and refer when appropriate to proper resource agencies on and off campus. 
  • Coordinate and track referrals, to insure continuity of care, to internal CSU units such as Student Disability Center, CSU Health Network, University Housing, Student Resolution Center, Student Legal Services, etc. 
  • Coordinate student transitions from CSU services to external, community providers (e.g. psychiatric/psychological treatment services or alcohol and drug treatment services etc.). 
  • Communicate with staff, parents, families, or designated emergency contacts regarding significant mental health emergencies including hospitalizations, suicide attempts, or other behaviors, and help facilitate a successful return to campus when issues are appropriately resolved. 
  • Seek immediate assistance from the Director, Associate Director, Assistant Director of Student Case Management and Referral Coordination, the Dean of Students, or the CSU Police in situations where there is a concern for the student’s safety or the safety of others.
  • Maintain accurate case management records. 
  • Provide training, educational, and consultative initiatives for campus constituents regarding case management services and procedures. 
  • Participate in departmental processes and projects.
  • Fulfill other duties as assigned.
  • Participate in university, divisional and departmental training and development activities. 
  • Attend local, regional or national workshops/conferences for continued development and training. 
  • Stay current in higher education, student leadership, diversity and social justice, etc. (through readings, webinars, trainings, workshops, conferences, etc.).
  • Partner with the SCM staff for selection, supervision and evaluation of graduate student practicums and internships, and student staff selection.

 

Conditions of Employment:

  • Pre-employment Criminal Background Check (required for new hires).
  • Valid Driver’s License - Must have a valid driver’s license or the ability to obtain a driver’s license or access to a licensed driver by the employment start date.
  • On-call Status.
  • Travel - local travel is required to meet students off campus and in hospitals.

 

Supervision:

This position has no supervisory duties.

 

Minimum Qualifications:

  • Masters Degree in Counseling, Psychology, Social Work, Student Affairs, or related field.
  • Strong commitment to, and experience in, working with people from diverse backgrounds and experiences.
  • Experience working with clients/students with mental health concerns, alcohol or drug issues and crisis situations.

 

Preferred Qualifications:

  • Knowledge about, and skills in, developmental, psychological and medical issues in the college student population.
  • Ability to work cooperatively with others in a multi-disciplinary team environment.
  • Excellent interpersonal skills including both oral and written communication.
  • Prior experience in a University setting.

Salary Range

$55,000 -- $59,000
