Victorian Post Acute is a 90-bed, rehabilitation-focused skilled nursing facility committed to helping residents achieve their highest level of independence and quality of life. We are currently seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our dynamic in-house therapy team.

Key Responsibilities

  • Evaluate residents and develop individualized treatment plans.

  • Provide skilled speech-language pathology services addressing speech, language, cognition, voice, and swallowing disorders.

  • Perform clinical swallowing assessments and implement dysphagia management programs.

  • Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.

  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.

  • Participate in interdisciplinary care conferences and discharge planning meetings.

  • Educate residents, caregivers, and staff on communication techniques, swallowing precautions, and home exercise programs.

  • Maintain compliance with all regulatory, facility, and documentation standards.

Qualifications

  • Current California Speech-Language Pathologist (SLP) license in good standing.

  • CCC-SLP preferred or eligibility as required by state regulations.

  • Strong communication, organizational, and interdisciplinary collaboration skills.
