What You’ll Do:
- Evaluate patients and develop individualized care plans.
- Provide direct speech-language pathology services in accordance with physician orders.
- Educate patients and caregivers on home therapy programs.
- Ensure accurate documentation and compliance with OASIS and PPS guidelines.
- Collaborate with physicians and interdisciplinary team members for continuity of care.
What We’re Looking For:
- Master’s Degree in Speech-Language Pathology.
- Current, unrestricted SLP license in the state of practice.
- 1+ year of clinical experience as an SLP.
- Ability to travel within Middlebury & Danbury coverage area (must have reliable transportation, valid driver’s license, and auto insurance).
- Comfortable lifting 50–100 lbs and working in a physically demanding environment.
