POSITION SUMMARY
The HCBS Care Coordinator provides assistance with establishing HCBS services for eligible individuals, performs record-keeping and administrative tasks related to program functions, and represents the Company in a professional and courteous manner at all times.
DUTIES AND RESPONSIBILITIES
- Provides phone support for incoming calls from HCBS clients/staff/referral sources and provides the necessary assistance to meet their needs/requests.
- Assists in review, implementation, evaluation, and education of HCBS staff/clients on regulations and policies for program compliance.
- Provides administrative support related to HCBS billing/payroll tasks.
- Maintains daily schedule with focus on organization and time management.
- Maintains clear, concise, and accurate records in relation to referrals, community contacts, requests for information, employment verifications, and other related tasks.
- Assists with information gathering and problem resolution when a client or employee concern/complaint is noted.
- Acts as a resource and support for HCBS staff/clients.
- Assists with phone support as needed.
- Performs other duties as may be deemed necessary by the HCBS/FMS Manager.
MINIMUM JOB REQUIREMENTS
- Must be eighteen (18) years of age or older (per state guidelines).
- Education/Licensure: High school diploma or equivalent.
- Experience: One year of experience in an office setting preferred. One year of experience in home care preferred.

Ability to work independently in a fast-paced, stressful environment, utilizing good judgment and patience in all situations. Position requires mental alertness, analytical ability, organizational skills, multitasking, problem-solving, communication, concentration, and initiative.
CONDITIONS OF EMPLOYMENT
- Successful candidate must have a satisfactory post-offer medical review and must pass a pre-employment criminal background check.
- Must have proof of auto insurance (minimum required by law).
- Candidate must meet the requirements of the full job description as stated throughout the duration of employment with PHC.

- Frequent sitting – extended periods

- May involve driving automotive equipment for business purposes

- Occasional pushing/pulling up to 50 pounds
- Fast-paced and occasionally stressful work environment with tight deadlines; exposure to mechanical and electrical office equipment.
- Employee may be required to adapt to various home conditions including, but not limited to, temperatures in excess of 80 degrees and below 55 degrees, humidity, working around client's family or pets, cigarette smoke, dust, or infectious disease.
