Senior Doc, part of Brigade Health & Affiliates, is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high-quality geriatric psychiatry and behavioral health services in home, assisted living, memory care, and skilled nursing settings.
If you’re passionate about senior mental health, dementia care, integrated care models, and autonomous practice, this is an opportunity to build and manage your own panel while collaborating with primary care, palliative care, and neurology teams.
Integrated, continuum-based behavioral health model
Focus on dementia-related behaviors, depression, anxiety, psychosis, mood disorders, and complex psychiatric comorbidities
Work within a collaborative interdisciplinary team
Autonomy with strong operational and clinical support
Clear pathway into clinical leadership and program growth for high-performing providers
Comprehensive psychiatric evaluations & medication management
Management of behavioral symptoms in dementia and cognitive decline
Collaboration with PCPs, palliative care, and facility partners
Caregiver education and relationship-based care
Documentation, care coordination, and panel management
Active PMHNP-BC license
Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry

This role is ideal for an NP or PA who is confident managing chronic cardiac conditions, values communication with referral partners, and thrives working independently while aligned with a strong clinical team.
What You’ll Do
• Independently manage hypertension, CAD, CVA, dyslipidemia (HLD), PVD, PAD
• Oversee and adjust anticoagulation therapy, including Coumadin
• Perform comprehensive cardiovascular assessments and medication management
• Coordinate with internal providers and external referral partners
• Maintain clear communication with community stakeholders
• Deliver evidence-based, outcomes-focused care
What We’re Looking For
• Active, unrestricted license in state of practice
• 2+ years of cardiology or advanced chronic disease management experience
• Strong diagnostic and clinical decision-making skills
• Confident independent operator
• Excellent communication and referral relationship management skills
