About the Opportunity
Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. We are looking for a highly motivated full-time Lead Care Manager who is willing to think creatively and empathically to help our team change the way people access healthcare.
We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide.
This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 2-3 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home
What You’ll Do
- Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals
- Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system
- Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members
- Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan
- Supports nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks
- Collaborates on care issues with Enhanced Care Management team by participating in systematic case reviews and consulting with nurse care manager, behavioral health care manager, and nurse practitioner before taking clinical actions
- Consistently meet monthly encounter metrics to ensure compliance with health plan regulations
- Identify and break down barriers ensuring individuals’ continuation with the program
- Assists individuals in securing connection to community supports by scheduling appointments, managing referrals, and ensuring timely follow-ups
- Coordinate physical care management appointments through collaboration with external and internal providers
- Utilize external and internal online platforms to collaborate with team members and carry out daily tasks
What You’ll Need
- 1+ years of general work experience (Case Management preferred)
- You are physically located in California
- Field Ops requires you to maintain reliable transportation for engagement at clinic, community based organization, and health system partner locations
- Virtual Ops requires a quiet, HIPAA compliant and internet connected space
- Bilingual – English/Spanish
- Strong understanding of cultural fluency
- High degree of empathy
- Ability to work collaboratively in a multidisciplinary team
- An eye for optimization
- Organizational skills
- Ability to remain patient when faced with adversity
- Strong technical skills and comfort with technology innovation, past experience with CRM databases, basic Excel, Word, email, and video conferencing
- A valid driver’s license and auto liability insurance

- Demonstrated professional or personal lived experience working closely with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder
- 2+ years of case management experience
- Experience with motivational interviewing
- Knowledge of medical terminology

- Detail-oriented and organized self-starter who is a rockstar multitasker

We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 2-3 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home
What You’ll Do
- Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals
- Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system
- Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members
- Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan
- Supports nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks

About the Opportunity
Pair Team is hiring a Bilingual Family Nurse Practitioner to support the launch of our Medicare ACCESS program across multiple states. This role will provide virtual care to Medicare patients with chronic, complex, and often overlapping medical, behavioral health, and social needs.
You will work as part of a centralized, multi-state clinical team, partnering closely with RNs, Lead Care Managers, and clinical leadership to complete patient assessments, guide care plans, support escalations, and help build a scalable care model for older adults. This role is 100% remote, but candidates must be comfortable working across EST and PST time zones as needed.
Key Responsibilities:
- Provide virtual care and clinical assessments for Medicare patients across multiple ACCESS markets
- Support initial patient assessments, care plan direction, and ongoing clinical oversight
- Manage chronic conditions, acute concerns, medication-related questions, and routine follow-ups through telemedicine
- Partner with RNs, Community Health Workers, and clinical leadership to close care gaps and ensure timely follow-up
- Provide clinical guidance during case reviews, escalations, and complex patient situations
- Support patients with conditions common in Medicare populations, including hypertension, diabetes, CKD, cardiovascular disease, chronic pain, depression, and anxiety
- Build trusted relationships with patients who may have limited access to consistent care
- Collaborate with operations and product teams to improve care delivery workflows, documentation, and patient engagement

What You’ll Need
- 2+ active Nurse Practitioner licenses in Pair Team’s planned ACCESS launch and expansion states, including California, Nevada, Pennsylvania, New York, Maryland, Georgia, West Virginia, Texas, Florida, or North Carolina
- Willingness and ability to obtain additional state NP licenses as Pair Team expands into new ACCESS markets
- Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner required
- Bilingual Spanish and English fluency required
- 2+ years of clinical experience as a Nurse Practitioner
- Experience caring for Medicare, geriatric, or medically complex adult populations

About the Opportunity
Pair Team is looking for an Account Executive to join our Partnerships team and with a primary focus on ACOs, physician groups and provider practice partnerships nationwide. This role is designed to capitalize on a strong product-market fit of Pair Team’s AI driven social care management in the ACO channel and take dedicated ownership of an active pipeline while expanding into new markets.
This is a true end-to-end individual contributor sales role for someone who knows how to build momentum in a fast-moving market, convert demand into closed deals, and independently generate new opportunities. We’re looking for someone who understands how ACOs and provider practices evaluate partnership opportunities, can speak credibly to how physicians generate value in ACO MSSP / REACH programs, and is comfortable balancing inbound conversion with outbound pipeline generation. You’ll report directly to our SVP, Networks and partner closely with our Director of Partner Engagement, with support from executive introductions and a Business Development Advisor on strategic accounts.
What You’ll Do
- Own the full sales cycle for our physician partnerships, from prospecting and outreach through discovery, proposal development, negotiation, and close
- Develop a deep understanding of the programs that fund our physician practice offering - Medicare ACCESS
- Take ownership of an active sales pipeline and drive opportunities forward to close while expanding into new markets nationwide
- Self-source approximately 50% of your pipeline through outbound outreach, networking, referrals, and strategic prospecting
- Convert inbound interest generated through marketing, webinars, content, events, and executive introductions into qualified opportunities and closed deals
- Build and manage relationships with executive, operational, and clinical stakeholders across ACOs, provider practices, and other prospective partners
- Navigate complex, multi-stakeholder sales cycles with strong follow-through, sound judgment, and attention to detail
- Maintain strong CRM discipline in HubSpot, including pipeline updates, activity tracking, next steps, and forecasting
- Develop a strong understanding of Pair Team’s ACCESS program and clearly communicate our value to external partners
- Partner closely with senior leadership, external advisors, and BD support to shape market strategy, account prioritization, and outbound approach
- Surface market feedback, buyer insights, and competitive intelligence to inform broader sales and partnerships strategy
- Represent Pair Team in partner meetings, events, and other external forums as needed
What You’ll Need
- 3 - 5 years of experience in account executive, business development, or full-cycle sales roles
- Direct experience selling to provider practices, ACOs, or adjacent value-based care organizations is required
- Strong track record of owning deals end to end, including prospecting, managing pipeline, negotiating, and closing
- Familiarity with ACO value drivers and core terminology such as MSSP tracks, HEDIS scores, MLR, care management, and shared savings models
- Demonstrated ability to self-source pipeline and independently create opportunities rather than relying entirely on inbound demand
- Experience balancing outbound prospecting with conversion of warm or inbound opportunities
- Ability to build trust and credibility with executive, operational, and clinical stakeholders
- Experience selling healthcare services, tech-enabled services, care models, or workflow solutions into provider organizations strongly preferred
- Experience using HubSpot, Salesforce, or another CRM with a high level of consistency and organization

Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. We are looking for a highly motivated full-time Lead Care Manager who is willing to think creatively and empathically to help our team change the way people access healthcare.
We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide.
This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 2-3 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home
What You’ll Do
- Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals
- Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system
- Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members
- Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan
- Supports nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks
- Collaborates on care issues with Enhanced Care Management team by participating in systematic case reviews and consulting with nurse care manager, behavioral health care manager, and nurse practitioner before taking clinical actions
- Consistently meet monthly encounter metrics to ensure compliance with health plan regulations
- Identify and break down barriers ensuring individuals’ continuation with the program
- Assists individuals in securing connection to community supports by scheduling appointments, managing referrals, and ensuring timely follow-ups
- Coordinate physical care management appointments through collaboration with external and internal providers
- Utilize external and internal online platforms to collaborate with team members and carry out daily tasks
What You’ll Need
- 1+ years of general work experience (Case Management preferred)
- You are physically located in California
- Field Ops requires you to maintain reliable transportation for engagement at clinic, community based organization, and health system partner locations
- Virtual Ops requires a quiet, HIPAA compliant and internet connected space
- Bilingual – English/Spanish
- Strong understanding of cultural fluency

We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. What You’ll Do
- Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals
- Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system
- Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members
- Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan

About the Role
As a Principal AI Engineer, you will help design and build the architecture that enables this evolution, translating complex care processes into reliable, production-grade AI systems in close partnership with product and domain experts.

About the Role
As a Staff AI Engineer, you will help design and build the architecture that enables this evolution, translating complex care processes into reliable, production-grade AI systems in close partnership with product and domain experts.
