About the role:
The Clinical Services Liaison serves as a key connection between referral sources, patients, families, and the hospice clinical team. The Clinical Services Liaison collaborates closely with hospital staff, physicians, case managers, and community partners to promote timely and appropriate hospice admissions while supporting the organization’s mission of delivering compassionate, patient-centered end-of-life care.

The Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Luminary Hospice, either directly or through coordination of care with other spiritual counselors.
1. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
2. Facilitates the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
3. Provides direct spiritual care to patients and families/caregivers.
4. Serves as liaison and support to community chaplains and spiritual counselors.
5. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
6. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
7. Attends patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary group.
8. Provides consultation, education, and support to the interdisciplinary group on spiritual care.
9. Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
10. Provides for funeral or memorial services for patients as requested.
11. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
12. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
13. Actively participates in quality assessment performance improvement teams and activities.
14. Accepts and performs other related duties and responsibilities as required.
1. Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver.
2. Ability to work as member of interdisciplinary group.
3. Comfort in an interfaith setting.
4. Ability to accept different lifestyles, cultures, beliefs, and values.
5. Ability to network with community clergy and congregations.

The Medical Director will serve as a hospice champion – promoting and representing the program to physicians, physician groups, discharge planners, other referral sources, community health organizations, and potential donors, as appropriate.

The PRN Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosis of medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, and coordinating and collaborating with patients, families, and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments, and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. About the role:
The after-hours registered nurse supports the patient needs of the agency in the evening during the weekdays, working overnight shifts Monday through Thursday (5:00 PM – 8:30 AM) and a continuous weekend shift from Friday at 5:00 PM through Monday at 8:30 AM. This role follows a rotating schedule: one week on, followed by one full week off. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities after normal business hours.

The PRN Hospice Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations, and documenting observations and care performed.
The PRN hospice aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
2. Providing patient and family with positive communication techniques, within the realm of ethical and respectful care, including confidentiality.
3. Meeting the safety needs of the patient and using equipment safely and properly (foot stools, side rails, O2, etc.)
4. Giving personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.

The PRN Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Luminary Hospice, either directly or through coordination of care with other spiritual counselors.
1. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
2. Facilitates the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
3. Provides direct spiritual care to patients and families/caregivers.
4. Serves as liaison and support to community chaplains and spiritual counselors.
5. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
6. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
7. Attends patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary group.
8. Provides consultation, education, and support to the interdisciplinary group on spiritual care.
9. Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
10. Provides for funeral or memorial services for patients as requested.
11. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
12. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
13. Actively participates in quality assessment performance improvement teams and activities.
14. Accepts and performs other related duties and responsibilities as required.
1. Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver.
2. Ability to work as member of interdisciplinary group.
3. Comfort in an interfaith setting.
4. Ability to accept different lifestyles, cultures, beliefs, and values.
5. Ability to network with community clergy and congregations.
6. Knowledge of and commitment to hospice philosophy of care.
7. Hospice/ experience preferred.
8. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations, and documenting observations and care performed.
The hospice aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
