About the Role
As a Senior Care Center Manager (Senior CCM), you will serve as a key operational leader responsible for overseeing multiple clinic sites (typically two) and driving performance, consistency, and culture across locations. This is a step beyond single-site management—you are accountable not just for execution, but for scaling best practices, developing leaders, and ensuring operational excellence across clinics and teams.
You will partner closely with clinical and operational leaders to ensure clinics deliver high-quality, high-touch care while meeting clinical, operational, and financial goals.
A successful Senior CCM operates as both a player-coach and systems thinker—able to zoom in on day-to-day challenges while also building the structures and people needed for long-term success.
What You'll Do
Lead Multi-Site Operations & Performance
- Provide leadership and foster collaboration across team members in Hopscotch clinics to deliver best-in-class patient care, support care teams and achieve goals around clinical outcomes, quality, operational and financial performance
- Partner with physician and clinical leaders to enable successful clinical, operational, and financial outcomes and ensure they have the appropriate support needed
- Collaborate with teams across the company to share clinic information, identify/resolve/escalate issues and concerns, and support on-going training and development
Build & Develop High-Performing Teams
- Manage, coach, and mentor multidisciplinary teams of professionals across clinic locations
- Build team member capability across clinics through coaching, feedback, and development planning
- Foster a culture of accountability, collaboration, and continuous improvement
- Support hiring, onboarding, and talent planning across sites
Support Best-In-Class Patient Care
- Advocate for patients and team members and foster a patient-centered, positive and supportive work culture
- Oversee care delivery for up to 1,000+ patients, ensuring patients receive superb care they would willingly refer others to in the community
- Collaborate with physician and clinical leaders to ensure alignment between clinical and operational priorities
- Ensure PCPs and care teams have the support needed to deliver high-quality care
- Help navigate complex operational or team challenges that impact care delivery
Drive Operational Excellence
- Ensure strong execution of core operational processes across all sites (scheduling, billing, workflows, compliance)
- Standardize and optimize processes to improve efficiency and patient experience
- Oversee clinic facilities and operational needs across locations
- Support and lead implementation of new initiatives, tools, and workflows
Own Financial & Metric Performance
- Manage and influence P&L performance across multiple clinics
- Track, analyze, and act on key performance metrics across sites
- Partner on budgeting, forecasting, and resource allocation
- Ensure clinics are meeting targets tied to value-based care performance
Ensure Compliance & Risk Management
- Maintain compliance across sites with all regulatory requirements (HIPAA, OSHA, CLIA, etc.)
- Identify and mitigate operational risks proactively
Impact Community & Growth Efforts
- Support growth across clinics, including patient acquisition and community engagement
- Build and maintain relationships with community partners
- Ensure each clinic is a strong ambassador for Hopscotch in its local market
About You
You’re a strong operator and people leader who knows how to scale what works. You’re comfortable owning outcomes across multiple teams and locations—and you don’t lose sight of the human side of leadership while doing it.
Experience
- 4+ years of leadership experience in a clinical or healthcare setting, including managing managers or multiple teams
- Experience overseeing multi-site operations strongly preferred
- Demonstrated success driving operational, financial, and quality performance
- Experience working in value-based care or complex care models preferred

What You’ll Do (And Why It Matters)
You’ll be a key connector between patients, providers, and the broader care team—ensuring every visit is smooth, thoughtful, and impactful.
Before & During Visits
- Welcome patients and set the tone for a positive, comfortable experience
- Prepare exam rooms and anticipate care needs (vaccines, labs, screenings)
- Room patients efficiently while capturing accurate, meaningful clinical info
- Take vitals, review medications, and identify patient needs
- Support providers so they can stay focused on care—not logistics
Clinical Support
- Perform phlebotomy and basic lab tests
- Administer vaccines and screenings (within certification)
- Assist with procedures and care delivery
- Coordinate diagnostics, DME, and follow-up care
Beyond the Visit
- Provide phone support and guidance to patients
- Collaborate in care team meetings to improve outcomes
- Help ensure continuity and follow-through on care plans
Clinic Excellence
- Maintain clean, safe, fully stocked exam rooms

About You

Qualifications
- High school diploma or GED
- Graduate of an accredited Medical Assistant program (or equivalent training)
- 1+ year healthcare experience (or clinical rotations)
- BLS/CPR certified
- Comfortable using EMR systems

What You'll Do
As the Medical Director, you will provide day-to-day leadership for the clinic teams and deliver high-quality care, an exceptional patient experience and a best-in-class culture, acting as a Champion of the Hopscotch Care Model and a Culture advocate for your team and your patients. Specifically, you will:
- Provide leadership, oversight and support for the team to drive day-to-day execution against critical priorities to enable high-quality care, a great patient experience, engagement across the team and a best-in-class culture
- Set the curve for Care team and across Care Centers through clear understanding and execution to deliver care in the Hopscotch care model, a commitment to results, quality and excellence and by executing against key workflows and processes to enable the model
- Management of Providers including ensuring all Providers complete administrative tasks and utilize indirect care hours appropriately, managing their panels and completing documentation
- Partner with the Care Center Manager to outperform against target metrics and outcomes, including patient experience, STARS/HEDIS, care coordination, utilization and cost of care, through collaboration and engagement of your clinical care team
- Provide high-quality patient care in the comprehensive and interdisciplinary care model of Hopscotch health to deliver best-in-class outcomes and results for your dedicated patient panel
- Provide direct oversight of your clinical care team, including Hopscotch Advanced Practitioner, Medical Assistant, Care Manager, Community Health Worker and others
- Support the Hopscotch mission and vision through your leadership around talent and culture, clinical innovation and compliance/best practices to enable success
- Represent Hopscotch Health in the community and provides community-based education, engagement, and participation in events such as health fairs/conferences
About You
You’ll be a great fit for this position if you have 5+ years of post-Residency experience and have experience in managing teams. You must be Board Certified in Internal Medicine or Family Medicine, have an active, non-probationary North Carolina state medical license (or willingness to obtain), and be authorized to work in the US. An ideal candidate will also have:
- Familiarity with innovative clinical models, specifically those that serve high acuity members
- Experience with the Medicare population, including in-home care as well as documentation and compliance requirements
- Experience caring for “at-risk” panel of patients
- Experience using data and insights to drive action in a clinical setting
- Experience leading, teaching, and training others, in both formal and informal ways
You also have a proven track record demonstrating:

What You'll Do
As an Advanced Practice Clinician, you will provide comprehensive and interdisciplinary primary care with a focus on patient outcomes, quality metrics, and patient experience. Specific responsibilities will include, but are not limited to:
- Deliver comprehensive primary care services to a set panel of patients, including office visits and telehealth visits for routine and acute primary care issues
- Work closely with physicians, community health workers, case managers, pharmacists, and others to deliver high-quality outcomes
- Completion of comprehensive annual wellness visits and risk assessments
- Practice with a “continuous-improvement” mindset, following evidence-based guidelines and innovating on care models that solve real problems for our patients
- Meet high benchmark for results on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization
About You
We are looking for an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) who has:
- A minimum of three years’ experience in a primary care specialty (internal medicine, geriatrics, family medicine)
- Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding and HRA (Health Risk Assessments)
- Experience working with multiple software platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills, and ability to manage multiple priorities appropriately
- An active, non-probationary state medical license
- US work authorization
