The Care Manager Registered Nurse (CMRN) is a hybrid role responsible for managing a panel of higher-acuity patients (HPP) through a combination of primarily remote case management and targeted in-clinic support, such as High-Risk Huddle meetings.

About the Role
Hopscotch Primary Care is seeking a mission-driven Psychiatric-Mental Health Nurse Practitioner (PMHNP) to support the behavioral health and psychosocial needs of our patients across Western North Carolina. As Hopscotch continues to grow and serve patients with increasingly complex behavioral health needs, this role will play a critical part in expanding access to high-quality, integrated behavioral health care across our clinics.
This role will be embedded in a team-based, primary care environment and focus on psychiatric assessment and diagnosis, medication management, and co-management of patients with behavioral health conditions. The Psychiatric NP will partner closely with primary care providers, care teams, and external behavioral health partners to deliver coordinated, patient-centered care.
This role offers flexibility in care delivery, including a mix of in-person and telehealth visits, and will involve travel to clinic locations across Western North Carolina.

ABOUT THE ROLE
Hopscotch Primary Care is seeking a clinically sound, mission-driven Licensed Clinical Social Worker to support the behavioral health needs of our patients. This role will be a key resource for primary care teams across the Western North Carolina region. As Hopscotch continues to grow and serve patients with increasingly complex behavioral health needs, this role will play a critical part in expanding access to high-quality, integrated behavioral health care across our clinics.
This role will work closely with primary care providers and care teams to deliver coordinated, patient-centered behavioral health care. What You'll Do
Hopscotch Primary Care is looking for a talented and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our teams in beautiful Western North Carolina. This role would provide float coverage at our clinic locations for a variety of needs. If you enjoy travel, variety of work and team members, and flexibility to work across multidisciplinary teams, this could be a great opportunity for you.
This position is full-time based in Western North Carolina and mileage reimbursement as well as travel expenses would be provided if assignments exceed more than one hour of travel time from your home. This role offers a base salary with additional guaranteed bonus opportunities.
