ABOUT THE ROLE
The Care Manager Registered Nurse (CMRN) is a hybrid role responsible for managing a panel of higher-acuity patients (HPP) through a combination of primarily remote case management and targeted in-clinic support, such as High-Risk Huddle meetings.
This role is accountable for end-to-end care management, with a strong focus on:
- Reducing avoidable admissions (ADK) and emergency department utilization (EDK)
- Improving clinical outcomes and patient experience
- Supporting care continuity across the healthcare continuum
The CMRN partners closely with providers, clinic staff, and Care Center Managers (CCMs) to deliver coordinated, proactive, and patient-centered care. This position is primarily remote, with regular in-office presence based on patient or program needs.
Specific responsibilities for this role will include, but are not limited to:
Panel Management & Care Coordination (Primarily – Remote)
- Manage a defined panel of high-risk patients, delivering comprehensive, longitudinal case management
- Develop, implement, and continuously update individualized care plans in collaboration with providers and care teams
- Perform ongoing telephonic outreach and monitoring to improve patient outcomes
- Coordinate care across the patients HPC provider, specialists, hospitals, EDs, SNFs, and community resources
- Partner and collaborate with transitions of care team, for a smooth transition and to ensure that the patient needs are met following the transitions of care period
Clinical Collaboration & Outcomes Management
- Partner with providers, MAs, LPNs, and Care Center Managers to align on patient care plans and priorities
- Escalate clinical concerns and barriers to care in real time
- Participate in team huddles, case reviews, and interdisciplinary care discussions
- Track and improve quality and utilization metrics tied to patient outcomes
In-Clinic Responsibilities (Hybrid Component)
- Maintain in-office presence minimum of 1 time a month and as needed to:
- Support high-risk patient visits
- Assist with care coordination for complex patients
Home & Community-Based Support
- Coordinate with in office LPN for occasional home visits for high-risk or complex patients when clinically appropriate
- Assess social determinants of health, home safety, and barriers to care
- Coordinate community-based services and resources to support patient care plan goals
Patient & Family Engagement
- Build trusted relationships with patients, families, and caregivers
- Provide education on disease management, medications, and care plans
- Utilize motivational interviewing and coaching techniques to drive behavior change
Program Quality, Compliance & Best Practices
- Adhere to care management protocols, regulatory requirements, and documentation standards
- Support continuous improvement of care management workflows and outcomes
- Identify and report gaps, risks, or adverse events
- Contribute to development of best practices, training, and process improvements
ABOUT YOU
You would be a great fit for this position if you have a minimum of 2 years of experience as a care manager embedded into an interdisciplinary team and the following:
- Active registered nurse (RN) license in North Carolina
- BLS certification
- Experience working in a primary care clinic focused on chronic disease management
- Experience with behavioral health and community-based organizations preferred
- Experience with motivational interviewing, behavior change, health promotion, and coaching

About the Role
Hopscotch Primary Care is seeking a mission-driven Psychiatric-Mental Health Nurse Practitioner (PMHNP) to support the behavioral health and psychosocial needs of our patients across Western North Carolina. As Hopscotch continues to grow and serve patients with increasingly complex behavioral health needs, this role will play a critical part in expanding access to high-quality, integrated behavioral health care across our clinics.
This role will be embedded in a team-based, primary care environment and focus on psychiatric assessment and diagnosis, medication management, and co-management of patients with behavioral health conditions. The Psychiatric NP will partner closely with primary care providers, care teams, and external behavioral health partners to deliver coordinated, patient-centered care.
This role offers flexibility in care delivery, including a mix of in-person and telehealth visits, and will involve travel to clinic locations across Western North Carolina.

Specific responsibilities will include, but are not limited to:
- Conduct comprehensive psychiatric evaluations and diagnostic assessments
- Provide psychiatric medication management, including initiation, titration, and monitoring of psychotropic medications

About You

About the Role
The Director, Regional Operations is a senior leadership role with full ownership for the performance, growth, and culture of a region of Hopscotch Primary Care clinics across Western North Carolina. This leader will directly shape how high‑quality, value‑based primary care is delivered in rural communities, driving measurable improvements in clinical outcomes, patient experience, team engagement, and cost of care.
The scope for this role includes:
- Leading a portfolio of 5–7 clinics across Western North Carolina
- Direct leadership of 6–9 direct reports, including Care Center Managers
- Full accountability for clinical outcomes, financial performance, growth, and culture
- Direct partnership with the Chief Operating Officer and enterprise leaders across Population Health, Finance, Growth, and People Operations
The Director, Regional Operations will be required to live in Western North Carolina (e.g., the Greater Asheville, NC or Charlotte, NC areas) and must be willing to travel in and around Western North Carolina frequently.

They are responsible for assessing the behavioral health needs of patients, making referrals to appropriate behavioral health resources and specialists and providing counseling and other treatments as necessary.
This role offers flexibility in care delivery, including a mix of in-person and telehealth visits, and will involve travel to clinic locations across Western North Carolina.

What You'll Do
Hopscotch Primary Care is looking for a talented and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our teams in beautiful Western North Carolina. This role would provide float coverage at our clinic locations for a variety of needs. If you enjoy travel, variety of work and team members, and flexibility to work across multidisciplinary teams, this could be a great opportunity for you.
This position is full-time based in Western North Carolina and mileage reimbursement as well as travel expenses would be provided if assignments exceed more than one hour of travel time from your home. This role offers a base salary with additional guaranteed bonus opportunities.
As a Float Advanced Practice Provider, you will provide comprehensive and interdisciplinary primary care with a focus on patient outcomes, quality metrics, and patient experience. Specific responsibilities will include, but are not limited to:
- Deliver comprehensive primary care services to patients, including office visits and telehealth visits for routine and acute primary care issues
- Work closely with physicians, medical assistants, licensed practice nurses, case managers, pharmacists, and others to deliver high-quality outcomes
- Assist in completion of comprehensive annual wellness visits and risk assessments
- Practice with a “continuous-improvement” mindset, following evidence-based guidelines and innovating on care models that solve real problems for our patients
- Meet high benchmark for results on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization
About You
We are looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) who has:
- A minimum of four years’ experience in a primary care specialty (internal medicine, geriatrics, family medicine)
- Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding and HRA (Health Risk Assessments)
- Experience working with multiple software platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills, and ability to manage multiple priorities appropriately
- An active, non-probationary state medical license
- US work authorization
- Ability to travel locally frequently with reliable transportation
