Role Scope: 

We are looking for a dynamic, service-oriented Interdisciplinary Team (IDT) Coordinator who is passionate about supporting elderly persons in their journey. Under supervision of the Center Manager, the IDT coordinator ensures that IDT responsibilities are executed efficiently and effectively, including scheduling, meetings/discussions, follow-through.

  • Support in ensuring that IDT decisions and care plans aligns with What Matters Most to each participant 
  • Coordinate with IDT to support with timely SDR decisions and notifications to participants. 
  • Confirm that all required documentation and letters are sent to participants in a timely manner and reflected in their chart. 
  • Performs related duties as assigned. 

Required Qualifications:  

• High School Diploma or equivalency 

• Minimum of two (2) years of administrative support experience 

• Experience using Microsoft Word, Excel, and PowerPoint 

• Previous experience coordinating and facilitating care conference meetings. 

• Proof of medical clearance for communicable diseases, including a TB test. 

• Proof of all immunizations are up to date. 

• Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.. 

• Preferred Qualifications: 

• A minimum of two years experience working with the frail or elderly.

Compensation:

• Under the direction of the Recreational Therapist or Activities Coordinator, this position will support the planning, coordination, and facilitation of daily activity programs designed to enhance the physical, cognitive, and social well-being of participants. 

• Assist in implementing recreational, therapeutic, and social activities aligned with individual interests, abilities, and care plans. 

• Engage participants in group and one-on-one activities, fostering a welcoming, inclusive, and positive environment.

Role Scope: 

• Supervise daily clinic operations within the PACE clinic, ensuring high-quality, participant-centered care

• Assists participants and caregivers by providing leisure education for activity modification or skill development and supplies for in-home use as needed. 

• Plans a variety of activities that include, but are not limited to, exercise, arts and crafts, celebrations, special events, recreational games, community outings, and cognitive groups. 

• Ensure participant needs are being met and appropriately and descriptively chart patient progress and behavior. 

• Develops and distributes a monthly calendar of activities. 

• Organizes community outings and special events at Day Center. Arranges transportation, entertainment, catering, and ADHC staff needed. 

• Functions as a member of the interdisciplinary team; attends team meetings and periodic reviews. Reports changes in participants’ baseline status to appropriate staff on a daily basis. 

• Maintains general order in activities area(s) including daily clean-up, routine supply inventory, ordering, and purchasing.  Maintains overall general budgeting. 

• Assesses involvement in activity for participants placed in group/nursing homes; may include direct visit with participants in these settings.  Works with nursing home and other community-based living facility staff to impact participants in these settings. 

• Completes documentation including initial assessments, periodic reviews, individual service plans, and other data on participants as directed by the interdisciplinary team. 

• Provides on-going support to all staff involved in dementia-specific programming related to the dementia disease process. 

• Transports participants within the ADHC and rearrange activity rooms as needed. 

• Maintains and promotes compliance with all regulatory requirements, reimbursement standards, organizational standards, and policies and procedures related to area of responsibility.  

• Completes annual competency requirements. 

• Consistently achieves high standards of accuracy in work product and documentation.

• A minimum of two years experience working with the frail or elderly. 

• Working knowledge of recreational therapy techniques and practices of the elderly including assessments, care plan development, monitoring, and evaluation.  Familiar with illnesses common to diverse populations including frail older, developmentally, or physical disabled adult populations. 

• Working knowledge of physical, psychosocial, and behavioral needs of the elderly population. 

• Demonstrate creative abilities to plan, develop, and implement varied activities to meet diversified needs.

Role Scope: 

In this role, you will be responsible for the management and delivery of direct nursing care to our participants in a variety of settings including, but not limited to, the clinic, adult day center, participant’s home, virtually, and SNF.  You will serve as a critical member of the Interdisciplinary Team (IDT) and work collaboratively to complete assessments and drive forward participant care plans as “quarterback” for your panel.

• Participate and facilitate  in Interdisciplinary Team (IDT) meetings by contributing insights from assessments, care plan recommendations, and care coordination in a collaborative spirit. 

• Conducts face-to-face nursing assessments that are inclusive of physical, psychosocial, and behavioral statuses in various settings, primarily in the Habitat center but also in–home. 

• In partnership with a medical provider, delivers personalized care for a panel of participants based on care plans. 

• Delivers and documents nursing interventions as agreed upon in participant's care plans, promptly and accurately responding to physician orders, and correctly administering medications and therapeutic interventions. 

• Provides case management longitudinally and during transitions of care. Proactively coordinates complex patient discharges, transfers, and immediate post-discharge needs with hospital and long-term care facility case managers  

• Coordinates all aspects of care delivery including medication management, medical equipment and supplies, and specialist and diagnostic referrals  

• Triages in the outpatient setting, which includes independently initiating therapies within scope of practice and collaboratively working with a medical provider to escalate care as needed. 

• Educates participants, caregivers including family members, and team members on how to personalize and carry out care plans. 

• Aid with all wound care (including complex wounds), IV (hydration, therapies), and any additional procedures within RN scope of practice identified to meet evolving participant needs. 

• Delegates tasks to MA and Licensed Vocational Nurses within their respective scopes of practice. 

• Remotely takes after-hour calls that are triaged on a rotating schedule. 

• Performs related duties as assigned. 

Required Qualifications:  

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred. 

• Unencumbered Registered Nurse (RN) license required. 

• Minimum 2-4 years of experience clinically caring for medically complex or older adults’ population as an RN.   

• Minimum 1-2 year's experience in case management.  

• Strong clinical acumen in chronic disease management and complex geriatric care. 

• Demonstrates experience in management of clinical interventions: wound care, IVs, phlebotomy, colostomy/ileostomy care, etc. 

• Proof of medical clearance for communicable diseases, including a TB test. 

• Proof of all immunizations are up to date. 

• Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment. 

• Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law. 

Preferred Qualifications: 

• Alzheimer’s certification is preferred.

Role Scope:

Role: 

We are seeking an outstanding physician leader who is highly motivated and passionate about improving quality of care for vulnerable populations. Role Scope:

• Coordinates all aspects of care delivery including medication management, medical equipment and supplies, and specialist and diagnostic referrals  

• Triages in the outpatient setting, which includes independently initiating therapies within scope of practice and collaboratively working with a medical provider to escalate care as needed. 

• Educates participants, caregivers including family members, and team members on how to personalize and carry out care plans. 

• Aid with all wound care (including complex wounds), IV (hydration, therapies), and any additional procedures within RN scope of practice identified to meet evolving participant needs. 

• Delegates tasks to MA and Licensed Vocational Nurses within their respective scopes of practice. 

• Remotely takes after-hour calls that are triaged on a rotating schedule. 

• Performs related duties as assigned. 

Required Qualifications:  

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred. 

• Unencumbered Registered Nurse (RN) license required. 

• Minimum 2-4 years of experience clinically caring for medically complex or older adults’ population as an RN.   

• Minimum 1-2 year's experience in case management.  

• Strong clinical acumen in chronic disease management and complex geriatric care.

• Lead the end-to-end automation and AI pilot lifecycle, from opportunity identification to final evaluation and recommendation for scale, integration, or sunset. 

• Proactively engaging for environmental scans to ensure awareness and identification of new innovations and solutions that could be value creating for our platform 

• Develop and maintain a structured framework for partner selection, pilot implementation, and performance evaluation.

Role Scope: 

The Receptionist is the first point of contact for visitors, clients, and employees. This role is responsible for managing the front desk operations, including answering phone calls, greeting guests, and handling incoming and outgoing correspondence. Role Scope: 

The Network Manager is responsible for the strategic development, execution, and ongoing management of Habitat Health’s provider network across assigned markets and service lines. This role translates organizational network strategy into operational execution while ensuring that the provider network supports comprehensive service delivery, regulatory compliance, and high quality participant care within the PACE model. 

The Network Manager oversees provider contracting, provider recruitment, onboarding, and ongoing provider relationship management while ensuring that contracted providers are successfully integrated into Habitat Health’s operational and clinical workflows. This role serves as a senior liaison between Habitat Health leadership, center operations, and external provider organizations and is responsible for ensuring that contractual terms, regulatory expectations, billing requirements, credentialing standards, and care coordination processes are consistently implemented across the network. 
 
In addition to direct provider engagement, the Network Manager is responsible for overseeing network adequacy planning, supporting market expansion initiatives, guiding complex contract negotiations, and managing internal network development processes. The role may also provide supervision and guidance to network specialists or analysts responsible for supporting contracting and provider operations. Drafting, negotiating, and managing provider contracts, amendments, single case agreements, and letters of agreement for traditional and non-traditional providers required to support a comprehensive PACE network. 

• Lead the development and execution of network strategy within assigned markets to ensure a comprehensive, compliant, and accessible provider network that supports the full scope of PACE services. 

• Identify network gaps, service capacity needs, and strategic provider partnerships based on participant utilization patterns, geographic access requirements, regulatory standards, and program growth projections. 

• Lead complex provider contracting negotiations and oversee the development of provider agreements, amendments, letters of agreement, and single case agreements for both traditional and non traditional providers. 

• Establish and maintain strategic provider relationships with key health systems, specialty providers, community based organizations, and ancillary service partners required to support comprehensive PACE care delivery. 

• Oversee provider recruitment efforts across critical service areas including specialty physician services, behavioral health, home based care, transportation, durable medical equipment, dialysis, infusion, dental, and other essential services. 

• Direct provider onboarding and integration activities to ensure providers understand the PACE model, authorization workflows, billing requirements, claims submission processes, and interdisciplinary care coordination expectations. 

• Serve as a senior escalation point for provider operational issues including claims disputes, contract interpretation, service delivery concerns, credentialing requirements, and participant care coordination challenges. 

• Monitor provider network performance and compliance with contractual obligations and regulatory standards and implement corrective actions or performance improvement strategies when needed. 

• Maintain oversight of provider and contract tracking tools to ensure accurate documentation, reporting, and regulatory readiness. 

• Support cross functional collaboration with clinical operations, finance, credentialing, compliance, and center leadership to ensure alignment between network development, operational workflows, and participant care delivery. 

• Support new market launches and expansion initiatives through proactive provider recruitment, contracting strategy development, and establishment of foundational network partnerships. 

• Provide guidance, mentorship, and operational oversight to network specialists or other team members supporting contracting and provider relations activities. 

• Identify opportunities to improve network development processes, contracting efficiency, provider onboarding, and operational integration across markets. 

Qualifications 

• Bachelor’s degree in healthcare administration, business administration, public health, or a related field required. 

• Master’s degree in healthcare administration, public health, business administration, or related discipline preferred. 

• Minimum 6 to 8 years of experience in provider contracting, network development, managed care, or health plan operations. 

• Demonstrated experience leading healthcare provider contract negotiations and managing provider networks within a managed care, integrated delivery, or value based care environment. 

• Strong understanding of Medicare and Medicaid reimbursement methodologies, provider billing practices, and managed care contracting structures. 

• Experience developing and managing provider networks that support multi service care delivery models. 

• Working knowledge of regulatory and compliance requirements related to provider participation within Medicare or Medicaid programs. 

• Proven ability to manage multiple provider relationships, contracts, and operational priorities across complex healthcare markets. 

• Strong written and verbal communication skills with the ability to translate contractual and regulatory requirements into operational guidance. 

• Demonstrated ability to build and maintain strategic relationships with health system leaders, community providers, and internal stakeholders. 

• Strong analytical and organizational skills with proficiency in Microsoft Excel, Word, and provider network management tools. 

• Ability to operate effectively in a rapidly growing organization and lead initiatives within evolving operational environments. 

Nice to have: 

• Prior experience working in a PACE program or with PACE providers. 

• Experience with nontraditional provider types such as home and community based services, transportation, DME, dental, behavioral health, dialysis, or infusion services. 

• Familiarity with delegated credentialing models and credentialing documentation requirements. 

• Experience supporting claims issue resolution or billing education for providers. 

• Exposure to network adequacy reporting or provider directory management. 

Compensation:

Role Scope: 

The Referral Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal.  They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals.  The referral coordinator is an integral part of the care team, working as a liaison between the patient, providers, utilization management and various departments to ensure timely access to specialty care.

• Provide participants with information on referral processes, appointment details, and collaborate with care team on any required pre-appointment preparations. 

• Maintain effective communication with healthcare providers to ensure timely updates on referral statuses.

• Prepare and submit reports on referral activities and outcomes. 

• Assist participants with understanding their PACE benefits related to referrals.

Role Scope: 

Under the direction of Habitat Health leadership, the Executive Director is responsible for policy implementation, planning, development, administration, management, and program monitoring of the Habitat Health program. 

Core Responsibilities & Expectations for the Role 

• Assumes overall accountability and responsibility for Habitat Health. 

• Supervises the Medical Director and other components of Habitat Health operations. 

• Ensures that the job responsibilities of all direct reports are defined and understood. 

• Responsible for development, implementation, and service delivery operations of the Habitat Health program. 

• Oversees budget development, monitoring, and compliance to ensure solvency and positive financial performance across all sectors of program operations. 

• Oversees changes in service delivery and staffing as required by development and growth of the program. 

• Oversees marketing, government, and public relations. 

• Oversees QAPI Program to ensure appropriate indicators are established and necessary outcomes are attained. 

• Coordinates creation of PACE provider application and coordinates completion of state readiness review and CMS on-site visits. 

• Assures the successful operational, financial, and clinical development of Habitat Health and the accrual of sufficient risk reserve funds. 

• Responsible for the development, implementation and operation of the Habitat Health program including establishment of PACE centers and all necessary participant services/programs, compliance with all applicable PACE, state and federal requirements and monitoring of all expenditures and expectations. 

• Responsible for fiscal management and operation of Habitat Health. 

• Disseminates information about the Habitat Health model; represents and promotes the public knowledge of PACE to the community. 

• Assumes leadership role in Habitat Health strategic planning and establishment of short-term and long-term goals. 

• Explores and expands program and funding options and develops potential resources. 

• Protects privacy and maintains confidentiality of employees, participant, and other sensitive information. 

• Provides administrative on-call coverage on a rotation basis. 

• Performs related duties as assigned. 

• Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. 

Required Qualifications:  

Education: 

• Master's Degree is preferred in a related field including but not limited to business administration, health care administration, nursing or social service. 

Experience: 

• A minimum of 8 years experience in healthcare delivery/operations. A minimum of 5 years experience in supervisory or administrative capacity. Experience in complex brick and mortar healthcare delivery; multidisciplinary healthcare service operations 

 Skills and Knowledge: 

• Thorough knowledge of current concepts, theories, and practices related to home and community-based care for elders and disabled adults. 

• Working knowledge of health care delivery and financing systems including Medicaid, Medicare, waiver programs, prospective and capitated systems, public health programs and HMO/Managed Care systems. 

• Knowledge of the community’s health care, social services, and aging networks as well as relevant federal and state systems. 

• Knowledge of the procedures and practices of planning, program development, coordination, administration, and fiscal management. 

• Knowledge of the principles, procedures, and techniques governing provider contract negotiations. 

• Skills in oral and written communication. 

• Skills in problem analysis, negotiation, and decision-making.

Compensation:  

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $160,000 - $200,000 plus 15% annual bonus and other eligible incentives. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
