About the Role:

We are seeking a Dedicated and Collaborative Obesity Medicine Physician to join our innovative, multidisciplinary weight management team. This new role is designed for an ABOM-certified obesity medicine physician with a strong interest in teaching and supporting Advanced Practice Providers (APPs) in providing high-quality patient care. The physician will provide direct clinical services and structured supervision for up to three APPs, helping to enhance the consistency, safety, and effectiveness of patient care across the team.

• Have an MD or DO degree, are board certified and diplomates of the American Board of Obesity Medicine (ABOM)

About the Role:

We are seeking Physicians specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team, and who are actively licensed in AL, TN, OK, MO, IN, SC or GA, and currently hold a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).

This new role is Modified FTE (32 or 36hrs/week) role, designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in working collaboratively with Endocrine Advanced Practice Providers (APPs) in providing high-quality patient care.

A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy.

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a full-time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.

What You Will Do:

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs).
• You have your own panel of patients, and the role could also involve collaborating with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes. This role could involve APP collaboration and supervision. 
• Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.
• Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.
• Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs.
• Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship.

About You:

• Required: Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
• Highly Preferred: Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications). 
• Required: Active and unrestricted Medical State License in either of the following states AL, TN, OK, MO, IN, SC or GA, and currently holding a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact)
• Minimum of 5 years as an attending physician in endocrinology.
• Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
• Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens.

About the Role:

We are seeking an experienced, highly organized, and proactive Executive Assistant and Office Manager to support our CEO, COO, and Chief Medical Officer while also owning the day-to-day management of our Boston headquarters. In this role, you will serve as a strategic partner, helping senior leadership stay focused, organized, and operating at peak effectiveness during an exciting period of growth.

This is a hybrid role for someone who is equally comfortable providing high-level executive support and ensuring that a physical office runs smoothly, professionally, and consistently. What You Will Do:

Executive Support

• Provide direct administrative support to Form Health’s CEO, COO, and Chief Medical Officer
• Manage complex calendars, including scheduling across multiple time zones and prioritizing high-impact meetings
• Coordinate travel, logistics, expense reports, and preparation materials for executive meetings
• Act as a liaison between executives and internal/external stakeholders, ensuring timely, professional communication
• Prepare agendas, presentations, briefing materials, and follow-ups for leadership meetings, board meetings, and company-wide events

Boston Office Management

• Own day-to-day management of Form Health’s Boston headquarters, ensuring the office is clean, organized, welcoming, and ready for employees, executives, guests, and visitors
• Maintain the Boston office to a high standard, proactively identifying and resolving facilities, supply, maintenance, and employee-experience needs
• Coordinate facilities issues with the landlord, building management, maintenance vendors, cleaning services, and other service providers
• Serve as the primary point of contact for Boston office operations, including office access, guest experience, supplies, conference rooms, mail, deliveries, and general workplace needs
• Handle incoming mail and deliveries to the Boston office, including routing items to the appropriate teams and coordinating virtual or offsite scanned mail as needed

The Registered Dietitian will be responsible for providing individualized, ongoing, comprehensive medical nutrition therapy to patients through our telehealth platform. Job Responsibilities:

Work closely with Form Health obesity medicine physicians and endocrinologists, PAs and NPs to provide intensive, ongoing medical nutrition therapy support to patients via regular video visits

• Establish a professional and effective relationship with all patients. This includes being non-judgmental and using interpersonal skills to connect with patients, providing them with care that is comprehensive, empathetic, and evidence based.
• Create customized nutrition plans for patients based on their individual needs
• Provide nutrition education and counseling around diabetes self-management (primarily Type 2 Diabetes), healthy eating, and behavior change

We are seeking a Dedicated and Collaborative Obesity Medicine Physician to join our innovative, multidisciplinary weight management team. This new role is designed for an ABOM-certified obesity medicine physician with a strong interest in teaching and supporting Advanced Practice Providers (APPs) in providing high-quality patient care. The physician will provide direct clinical services and structured supervision for up to three APPs, helping to enhance the consistency, safety, and effectiveness of patient care across the team.

• Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will collaborate with APP colleagues to co-manage a shared panel and not have their own primary patient panel.
• Review charts and conduct return visits for APP patients every third visit to ensure quality and continuity of care.

• Prescribe non-controlled obesity medications for patients under both direct care and in coordination with APPs.

• Have an MD or DO degree, are board certified and diplomates of the American Board of Obesity Medicine (ABOM)

• If you don''t have a MA or NY State License, you MUST be licensed in any of the following states ME, NH, CT, NJ, DE, MD, DC, PA  AND currently holding a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).

About the Role:

We are seeking a dedicated and collaborative Physician specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. This new role is designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in supporting endocrine Advanced Practice Providers (APPs) in providing high-quality patient care.

The physician will currently provide direct clinical services and structured supervision for up to three Endocrine-Specialized APPs. A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy.

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a full-time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.

What You Will Do:

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs).
• Collaborate with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes, to co-manage a shared patient panel. 
• Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.
• Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.
• Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs.
• Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship.
• Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations.

About You:

• Required: Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
• Required: Active and unrestricted medical license in at least one of the following states: CA, TX, IL, MA, NY, or in any IMLC-participating state with a valid Letter of Qualification (LOQ) and ability to obtain additional state licenses
• Required: Current or prior credentialing with insurance payers
• Highly Preferred: Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications). 
• Minimum of 5 years as an attending physician in endocrinology.
• Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
• Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens.
• Strong interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes.
• Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.

About the Role:

We are seeking a dedicated and collaborative Nurse Practitioner (Advanced Practice Practitioner, APP) specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. This new role will involve using a lifestyle and obesity medicine approach to prevent and treat cardiometabolic conditions including comprehensive diabetes management.

The majority of your patient population will primarily focus on obesity/diabetes management across the full spectrum of disease, including Type 1, Type 2, post-transplant, and other complex secondary forms of diabetes (ex steroid-induced, 2nd to pancreatic disease, Cystic fibrosis related, MODY, etc). Endocrine APPs will help enhance the consistency, safety, and effectiveness of patient care across the newly formed endocrine team, specifically integrating the use of Continuous Glucose Monitors (CGMs), complex insulin regimens, and insulin pump therapy.

This is a full-time position based in a remote, telemedicine clinical setting. The APP will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.

What You Will Do:

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including titration of insulin and advanced use of diabetes technology (CGMs/pumps).
• Collaborate with Endocrinologist colleagues and Registered Dietitians specializing in diabetes treatment to co-manage a shared patient panel. 
• Review charts and conduct return visits for patients to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.
• Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.
• Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with Endocrinology collaborators.
• Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with collaborating physicians to review cases, address clinical challenges, and provide ongoing support.
• Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations.
• Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine.
• Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
• Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.

About You:

• Required: at least 2 years practicing advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens, able to sign a collaboration agreement with an Endocrinologist 
• Required: an active and unrestricted medical license- multi-state licenses are a plus.
• Preferred: OMA certification (CAEOM) or demonstrated clinical experience in obesity medicine (including older generation oral OMMs and newer injectable medications). 
• Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
• Experience delivering telemedicine care with excellent communication and interpersonal skills.
• Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity.
• Committed to delivering patient-centered, culturally competent care.

• Have a Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
• Are licensed to practice and prescribe legally in CA, CO or WA 
• RN Compact license or multistate license is a bonus
• Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting
• Have excellent interpersonal skills for patient and team interactions
• Are committed to the highest quality of medical care and an evidence-based approach to obesity management 
• Have prior experience conducting telemedicine visits and who have a great "web-side" manner

About the Role

We're looking for a strategic, hands-on, experienced Product Manager to lead Scheduling & Care Access product development at Form Health. Scheduling sits at the intersection of patient access, provider utilization, and operational scale. It’s how patients access care, how providers fill their days, and how we grow without losing the human moments that define our experience. This is a high-leverage role with end-to-end ownership of one of our most consequential product areas, including the authority to make build vs. buy decisions and shape the future architecture of our scheduling stack. You'll partner with engineering, design, operations, and clinical leaders to define the roadmap and deliver against it. You'll be joining a talented, experienced team and iterating on existing tooling with a bias toward data driven decisions and action.

What you will do

You will own Scheduling & Care Access end-to-end, including provider availability, matching, and the scheduling experience across both patient-facing and internal workflows.

Specifically, you will:

Understand and define the problem space

• Develop a deep understanding of workflows across patients, schedulers, patient coordinators, and providers
• Identify the highest-impact jobs-to-be-done to improve access while reducing operational burden
• Use data to uncover bottlenecks and opportunities across the scheduling lifecycle

Own product strategy and outcomes

• Define the vision, strategy, and roadmap for Scheduling & Care Access in alignment with company goals
• Set and drive key metrics such as utilization, time-to-first-appointment, no-show rates, provider load balance, and conversion through scheduling flows
• Continuously iterate based on data, experimentation, and feedback

Drive build vs. buy decisions

• Lead evaluation of third-party scheduling solutions vs. in-house development
• Prototype, test, and validate approaches, making recommendations grounded in data and long-term architectural fit
• Partner with engineering to shape the evolution of our scheduling platform

Deliver iteratively with engineering and design

• Partner closely with engineering and design to ship, learn, and iterate quickly
• Prioritize work to maximize impact and reduce feedback cycles
• Ensure a high-quality, scalable experience across both patient and internal tools

Collaborate across teams

• Translate business and user needs into clear product requirements, user stories, and functional specifications
• Lead demos, training, and communication across operations, clinical, commercial, and executive stakeholders
• Collaborate on adjacent roadmaps where scheduling intersects with other product and engineering teams

About you

You're a senior product manager who has lived in marketplace, scheduling, or logistics products. The kind where supply, demand, and matching all have to work, and where a small change in one variable ripples through the whole system. You've worked in healthcare or care delivery long enough to understand the intricacies of clinical scheduling and balancing that with a growing patient population.

Above all, you let data lead with small batch iterative delivery. You're equally comfortable making the call on build vs. buy, defending it with evidence, and going deep enough into APIs and integration workflows to know whether a vendor's promises hold up. Specifically, you bring:

• 7+ years of product management experience, with time on scheduling or logistics products.
• Healthcare or care delivery experience. You understand clinical workflows, eligibility, and the constraints that make healthcare scheduling materially different.
• A bias for driving product decisions with metrics and analytics.
• Comfort making and defending build vs. buy decisions, including evaluating third-party vendors and integrating across systems.
• Ability to dive into technical problems (APIs, multi-system workflows, integrations across homegrown and third-party software) and communicate clearly across functions in a remote-first environment.
• A track record of defining product vision, rallying support around it, and delivering iteratively against that vision.
• Self-driven, adaptable, and excited to learn in a rapidly growing startup.
• A genuine excitement for making compassionate, evidence-based healthcare accessible.
• You believe in small-batch delivery and getting value into users' hands sooner rather than later. You've seen what large releases cost in feedback latency and risk.
