Title: Dietary Aide
Job Summary: The Dietary Aide supports all dietary or culinary requirements in service of the dietary needs of our clients in recovery. They will be expected to participate in all appropriate mandatory in-service classes. They will follow the work schedule as assigned by the supervisor and perform other position-appropriate tasks as assigned. The Dietary Aide shall fulfill these responsibilities in a timely manner so as to meet the daily mealtime service requirements.

Title: Service Coordinator (Mental Health Case Manager)
Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

About the Campus: The Crestwood Center San Jose Psychiatric Health Facility (PHF) on our San Jose campus is in close proximity to the mental health support services in Santa Clara County. The San Jose PHF provides a spacious environment, with two living rooms, a welcoming room, visiting space, serenity room, dining room and a beautiful courtyard. The program is dedicated to restoring a sense of hope, self-empowerment and recovery in each of our clients. The San Jose PHF is designed for clients who require stabilization at a time of acute psychiatric crisis. We provide medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education and a thorough transition plan to enable a successful return to the community. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay.

Job Duties: The Dietary or Kitchen Manager directs and coordinates all aspects of the dietary or culinary services department. About the Campus: Our Skilled Nursing Facility is just minutes from the freeway, conveniently located by many shops and restaurants. As Neuro-behavioral campus we are less medically acute and as such there are great opportunities for clients. For example, Crestwood Wellness & Recovery Center has adopted the city park next to our building. Our clients actively pursue their gardening skills and creative beautification inspirations by maintaining and adding to the garden beds created on Earth Day 2009. The efforts have created a space for quiet and serenity as well. Weather permitting, staff often take clients to the park for group therapy, meditation, and yoga.

We provide medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education and a thorough transition plan to enable a successful return to the community. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay.

Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. Title: Mobile Crisis Licensed Vocational Nurse (LVN) | Licensed Psychiatric Technician (LPT)
Job Duties: The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) ensures we are providing safe, effective, and compassionate responses to individuals during a mental crisis. The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) will respond to calls from dispatch involving a mental health crisis in various settings. They will be required to travel to the location of the caller and provide crisis intervention services, including assessing and documenting holds. The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) will also be responsible for the supervision of the Peer Specialist as part of the Mobile Response Team (MRT).
About the Mobile Response Team: The Crestwood Behavioral Health Mobile Response Team (MRT) provides community-based interventions for those experiencing a mental health crisis. Our team is available to deploy into the community to provide crisis intervention, de-escalation, and stabilization. The MRT is staffed by Licensed/Registered Mental Health Counselors and Peer Specialists who can provide services throughout Napa County including residences, parks, shelters, schools, and hospitals. The goal of MRT is to de-escalate the crisis and safely connect individuals with mental health care and resources.

Title: Quality Assurance Coordinator
Job Duties: The Quality Assurance (QA) Coordinator plays an integral part in the overall functioning of the facility by monitoring and auditing the identified standards of person-served care as well as the quality of the entire electronic health record. Title: Director of Recreation | Recreation Therapist
Job Duties: The role of the Director of Recreation is to create and lead the recreational program with recreational activities, hobby & interest building, physical activities, and other events & holidays throughout the year. In our holistic approach, recreation is just one facet of the wellness of our clients, along with behavioral skill building, medication support, dietary teaching, and pre-vocational training. Our overall goal is to support our clients to create lives worth living and be successful as independents in their community.
