Title: Dietary Aide

Job Summary: The Dietary Aide supports all dietary or culinary requirements in service of the dietary needs of our clients in recovery. They will be expected to participate in all appropriate mandatory in-service classes. They will follow the work schedule as assigned by the supervisor and perform other position-appropriate tasks as assigned. Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

About the Campus: The Crestwood Center San Jose Psychiatric Health Facility (PHF) on our San Jose campus is in close proximity to the mental health support services in Santa Clara County. The San Jose PHF provides a spacious environment, with two living rooms, a welcoming room, visiting space, serenity room, dining room and a beautiful courtyard. The program is dedicated to restoring a sense of hope, self-empowerment and recovery in each of our clients. The San Jose PHF is designed for clients who require stabilization at a time of acute psychiatric crisis. We provide medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education and a thorough transition plan to enable a successful return to the community. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay.

Job Duties: The Dietary or Kitchen Manager directs and coordinates all aspects of the dietary or culinary services department. About the Campus: Our Skilled Nursing Facility is just minutes from the freeway, conveniently located by many shops and restaurants. As Neuro-behavioral campus we are less medically acute and as such there are great opportunities for clients. For example, Crestwood Wellness & Recovery Center has adopted the city park next to our building. Our clients actively pursue their gardening skills and creative beautification inspirations by maintaining and adding to the garden beds created on Earth Day 2009. The efforts have created a space for quiet and serenity as well. Weather permitting, staff often take clients to the park for group therapy, meditation, and yoga.

Title: Maintenance Assistant

Job Duties: The Maintenance Assistant is responsible for providing special projects in the maintenance department, carrying out planned maintenance programs as directed by the Director of Maintenance and documenting as required, expediting repair requisitions on a safety-first, patient-need, priority basis, repairs, replenishing, and refinishes walls, wall coverings, ceilings, and floors, and observes mechanical/electrical devices in operations, locating, adjusting, and correcting potential problems.

Job Duties: The Mobile Crisis Quality Assurance Coordinator plays an integral part in the overall functioning of the facility by monitoring and auditing the identified standards of person-served care as well as the quality of the entire electronic health record. The Mobile Crisis Quality Assurance Coordinator will ensure quality improvement standards are up to date and in compliance with the facility's level of care. This position encompasses aspects of staff development, infection control, medical records, and quality improvement to effectively collaborate with the QA committee and program at the facility level. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay. Our Residential Care for Elderly (RCFE) supports independence, while providing health and wellness programs, managing both physical and behavioral problems, and promoting integration into the community.

Title: Recovery Coach (Entry-Level Mental Health Aide)

Job Duties: Peer Support Specialists are people in a peer-designated role providing direct services with a personal experience of recovery from mental health, substance use, or trauma conditions who receive specialized training and supervision to guide and support others who are experiencing similar mental health, substance use, or trauma issues toward increased wellness.  A Peer Support Specialist is an individual who has made a personal commitment to his or her own recovery, has maintained that recovery over a period of time, has taken the 80-hour Peer Support Learning training (or equivalent) to work in mutuality with others, and is willing to share what he or she has learned about recovery in an inspirational way.

Title: Mobile Crisis Licensed Vocational Nurse (LVN) | Licensed Psychiatric Technician (LPT)

Job Duties: The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) ensures we are providing safe, effective, and compassionate responses to individuals during a mental crisis. The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) will respond to calls from dispatch involving a mental health crisis in various settings. They will be required to travel to the location of the caller and provide crisis intervention services, including assessing and documenting holds. The Mobile Crisis Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) will also be responsible for the supervision of the Peer Specialist as part of the Mobile Response Team (MRT).

About the Mobile Response Team: The Crestwood Behavioral Health Mobile Response Team (MRT) provides community-based interventions for those experiencing a mental health crisis. Our team is available to deploy into the community to provide crisis intervention, de-escalation, and stabilization. The MRT is staffed by Licensed/Registered Mental Health Counselors and Peer Specialists who can provide services throughout Napa County including residences, parks, shelters, schools, and hospitals. The goal of MRT is to de-escalate the crisis and safely connect individuals with mental health care and resources.

  • Bachelor’s degree and/or licensed nurse/licensed clinician. The program director shall also have at least two (2) years of experience or training in a mental health setting, one year of which shall include experience or training in program development for persons with severe and persistent mental disabilities.
• As an alternative to the bachelor’s degree and experience list above, a total of four years of experience in providing direct services in the community to persons with mental illnesses, of which 2 years must be in a position of supervising direct care staff, and graduation from high school or possession of a GED may be substituted.

Job Duties: The Quality Assurance (QA) Coordinator plays an integral part in the overall functioning of the facility by monitoring and auditing the identified standards of person-served care as well as the quality of the entire electronic health record. The QA Coordinator will ensure quality improvement standards are up to date and in compliance with the facility's level of care. This position encompasses aspects of staff development, infection control, medical records, and quality improvement to effectively collaborate with the QA committee and program at the facility level. The QA Coordinator is responsible for monitoring overall compliance with these aspects as they relate to current licensing and accrediting policies and regulations.

• A degree in a health-related field and/or two years’ experience in health care education.

Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. Qualifications:

• A minimum of 2 years of experience in Psychiatry and in working with a multi-disciplinary team is preferred.
• Current California Medical License, with a completed 3-year graduate training in Psychiatry from an approved AMA or AOA program.
• Experience with an In-Patient setting.

The per diem rate is $2,400 – 3,000 per day (6-10 hours per day with potential after-hours calls).
