Position: Licensed Vocational Nurse (LVN/LPN)
Reports To: Director of Patient Care Services
Schedule: Thursday – Saturday, Day and Graveyard available
As a Licensed Vocational Nurse at Cabrillo Hospice, you will provide compassionate, direct patient care under the supervision of a registered nurse. You will follow individualized care plans, administer treatments, and collaborate with our interdisciplinary team to ensure optimal patient outcomes.
Deliver direct patient care in accordance with the State Nurse Practice Act.
Implement nursing practices based on comprehensive patient assessments and care plans.
Document patient care accurately and promptly.
Provide patient and family education regarding diagnoses, treatments, and self-care.
Coordinate with hospice services, including aides, OT, PT, MSW, ST, dietitians, and supervisors.
Use equipment and supplies efficiently and effectively.
Manage pain and other symptoms, evaluating patient responses to treatments and medications.
Participate in personal and professional growth, quality improvement initiatives, and team development.
Perform other duties as assigned by the supervising registered nurse.
Graduate of an accredited practical or vocational nursing program.
Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience preferred.
Current LPN/LVN license in the state.
Committed to professional standards and nursing best practices.
Strong communication, organizational, and interpersonal skills.
Valid driver’s license with insured, reliable vehicle.
Current CPR certification.
Understanding of hospice philosophy and the needs of terminally ill patients.

We are seeking an experienced and relationship-driven Hospice Account Executive to join our San Diego team.
The Hospice Account Executive plays a critical role in managing and strengthening relationships with referral partners while ensuring continuity of care for patients transitioning into hospice services. This role focuses on account management, education, care coordination, and collaboration with internal clinical teams—not sales quotas.
Relationship & Account Management
Develop, manage, and maintain strong relationships with assigned referral partners, including hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare providers.
Serve as the primary point of contact for referral sources, ensuring consistent communication and responsiveness.
Care Coordination
Collaborate closely with discharge planners, case managers, social workers, and clinical teams to ensure smooth and timely patient transitions into hospice care.
Monitor referral processes to ensure patient needs, expectations, and care plans are clearly understood and met.
Education & Support
Provide ongoing education to referral partners regarding hospice eligibility, services, benefits, and care coordination processes.
Support referral sources by addressing questions, concerns, and operational challenges related to hospice services.
Internal Collaboration
Work alongside the interdisciplinary hospice team to align patient care goals, improve referral workflows, and enhance service delivery.
Participate in team meetings, care coordination discussions, and performance reviews.
Quality & Relationship Improvement
Identify opportunities to improve communication, service delivery, and partner satisfaction.
Address issues promptly and professionally to maintain trust and long-term partnerships.
Community Representation
Represent Cabrillo Hospice at community meetings, healthcare events, and professional gatherings as a trusted clinical and operational resource.
Experience in hospice, home health, or healthcare account management strongly preferred.
Solid understanding of hospice philosophy, eligibility criteria, and interdisciplinary care.
Minimum of two (2) years of experience working with physicians, hospitals, insurance providers, skilled nursing facilities, or other healthcare partners.
Strong communication, presentation, and relationship-building skills.
Excellent organizational skills with the ability to manage multiple accounts and priorities.
Ability to work independently and collaboratively within a multidisciplinary team.
Valid driver’s license and reliable transportation.
