Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.
The Hospice Triage RN will receive phone calls from Hospice patients and families. They will assess the needs of the patient and family, and coordinate the needed care and/or scheduling of home visits as needed. The triage nurse will collaborate with the transdisciplinary team members and facilitate communication among the transdisciplinary team, Hospice, and the patient’s Attending Physician.
Schedule:
Per diem; must be available at least 6 shifts per month; 8:30am - 5:00pm
Location:
On-site in Larkspur, CA

The Hospice Physical Therapist (PT) provides skilled physical therapy services to patients in their homes, helping them maintain independence, maintain safe mobility, and enhance quality of life. The PT develops individualized treatment plans, delivers evidence-based interventions, and collaborates with an interdisciplinary team to ensure coordinated, patient-centered care.
Schedule:
Per diem; 2 days a week or 6 shifts per month; 8:30am - 5:00pm
Location:
Marin County with coverage to Sonoma and S.F. Counties as needed
