Ensures standards of patient care are utilized and delegates responsibilities as defined by policy or necessary to ensure appropriate resident care.  From resident assessment, reviews and revises the care plan needs, problems and goal changes with the Charge Nurses.  Identifies necessary and appropriate nursing interventions pertinent to the plan of care.  Implements and documents all resident/family teaching relevant to health care needs and ensures understanding of the teaching.  Actively participates in the direct delivery of patient care.  Answer residents call light and assists with resident ADL’s as needed.  Completes rounds to ensure that residents are receiving the appropriate nursing care in an appropriate time frame.  Ensures that a safe, comfortable, clean environment is maintained.   Completes monthly QAPI, daily nursing labor forms for Administration, and monitors pharmacy recommendations monthly.  Attend Team Leader meetings twice per month and CNA meetings monthly.  Maintains working knowledge of emergency procedures and demonstrates awareness of safety issues relevant to patient and employee safety.   Handles crisis situations in a prompt, precise and professional manner.  Coordinates activities and assignments throughout the facility to ensure appropriate functioning of each neighborhood.  Analyzes problems, offers recommendations, and initiates problem resolution in a professional and timely manner.  Investigates employee concerns or incidents and takes the appropriate action.  Communicates to the Director of Nursing action taken and seeks advice in necessary situations.  May be assigned other duties as appropriate.

Qualifications

• Current RN license in the state you will be working
• Two years of long-term care supervisory experience preferred
• Three to five years of long-term care experience or similar nursing experience
• Excellent communication skills to interact verbally with residents, visitors, and staff
• Ability to prioritize, multi-task, manage time, and organize daily schedule to meet productivity standards
• Ability to stand and walk for long periods of time
• Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
• Ability to transfer and move residents who vary with ability to assist weighing 200+lbs using proper lifting techniques and with assistance as necessary
• Ability to push and pull equipment weighing in excess of 40lbs using proper techniques and with assistance as necessary
• Manual dexterity, coordination and skillful use of hands when working with residents and equipment
• Ability to perform necessary documentation
• Visual acuity
• Must be able to manage a diversity of activities

Summary

Participates in providing goal-oriented and appropriate group and/or person-centered sessions designed to meet the social service needs of all residents. Helps in addressing resident and family issues and acts as a resource to residents who are adjusting to a long-term care environment. Maintains availability to the residents in time of need, adjustments, lost articles, and concerns that they may want addressed. Participates in the planning of effective individualized social service programming through written assessment, multi-disciplinary treatment participation, and completion of required documentation. Ensures that charting is completed. Maintains resident rights through social service observation and assessment, while ensuring advance directives are reviewed upon admission and updated annually. Ensures that all residents are aware of the Resident Rights and have received a copy upon admission. In collaboration with interdisciplinary team, addresses needs of residents and family members within scope of practice. Develops relationships with residents and family members and addresses concerns professionally and in a timely manner. Provides social service assessments upon admission and for residents who have been hospitalized. Obtains new medical treatment form as needed and signed by resident's representative. Assists resident's representative with mail, paperwork, and Medicaid application as necessary. Aids in scheduling appointments for residents with physicians and outside needs. Follows up with resident's representative in regard to scheduling of appointments and appropriate transportation. Works in conjunction with interdisciplinary team for addressing all residents' needs. Contributes in the completion of admission, quarterly and annual notes to meet residents' physical and emotional needs. Participates in Resident Care Plan meetings. Addresses issues of resident's strengths and weaknesses and the social service needs of each resident in written form to aid in development of appropriate care plan. May be assigned other duties as appropriate.

Qualifications

• High school degree or equivalent
• Bachelor's degree in social work preferred
• Must have knowledge of growth and development and show expertise for geriatric population
• Certification as a Social Service Designee by State preferred
• One year experience in long-term care social service department preferred
• Previous geriatric and social work experience preferred
• Ability to perform necessary documentation in writing and electronically
• Manual dexterity and skillful use of hands when working with patients and equipment
