Transition Nurse (LVN)
Health Link Home Health & Hospice
Location: San Francisco, CA
Position: Full-Time | On-Site with Community Visits
Coverage: Supports patients across our San Francisco, East Bay, and South Bay branches
Compensation: $40.00–$45.00 per hour, commensurate with experience
About the Role
Health Link Home Health & Hospice has proudly served the Bay Area for nearly 20 years, empowering individuals and their families through compassionate, individualized care.
We are seeking a compassionate and organized LVN to join our newly established Transitions Department.
Based in our San Francisco office, this role supports our San Francisco, East Bay, and South Bay branches by helping Home Health patients navigate changes in their healthcare needs. You’ll work with patients, families, physicians, hospitals, and branch teams to coordinate appropriate services and facilitate smooth transitions throughout the continuum of care.
What You’ll Do
- Evaluate Home Health patients for the most appropriate level of care under the direction of the RN Transitions Nurse
- Coordinate transitions to Hospice, Palliative Care, Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), Board and Care homes, caregiver services, and other community resources
- Educate patients and families on available care options and resources
- Follow up with patients, caregivers, physicians, hospitals, and community providers to support successful transitions
- Identify and help resolve barriers that may delay patient care
- Work closely with Clinical Managers, Clinical Coordinators, Intake, Scheduling, Quality Assurance, and field clinicians to coordinate patient care
- Build and maintain relationships with physicians, hospitals, facilities, and community partners
- Maintain accurate documentation and comply with agency policies and regulatory requirements
What We’re Looking For
- Active California LVN license
- Home Health experience preferred
- Strong communication and organizational skills
- Ability to build positive relationships with patients, families, and healthcare providers
- Comfortable working independently and as part of a collaborative team
- Proficient with EMR systems and basic computer technology
- Valid California driver’s license and reliable transportation
