Sales Support Coordinator
About this Role:
This role is the critical link between referral sources and patient care - supporting our Home Health Account Executives by ensuring every referral is complete, every discharge is tracked, and every patient starts care without delay. You'll coordinate directly with hospitals, clinics, and physician offices to obtain discharge dates, orders, and paperwork through EMR portals, while also speaking directly with patients to coordinate care needs. Your work directly protects the speed and accuracy of our intake process - every referral you complete means a patient gets care faster and our AEs can focus on growing referral relationships. how it will impact the company.
Responsibilities:
· Serve as the primary point of contact for coordinating home health referrals on behalf of assigned Account Executives.
· Make outbound calls directly to patients and family members to coordinate start-of-care scheduling, gather intake information, and address questions or concerns.
· Communicate directly with physician offices, and clinics to track down and complete referral documentation.
· Navigate multiple EMR/health system portals (e.g., Homecare Homebase, WellSky, Epic, Cerner) to obtain discharge dates, physician orders, face-to-face documentation, and other required paperwork
· Monitor and follow up on pending referrals to ensure nothing is delayed or dropped, escalating as needed to keep patients on track for timely admission.
· Maintain compliance with HIPAA and patient privacy standards on all patient communications and documentation handling.
· Support administrative and coordination tasks as needed to ensure smooth transition of patients from referral to admission
Required qualifications:
· Background in home health, hospice, DME, discharge planning, case management, or physician office referral coordination.
· Demonstrates organization/follow-through managing multiple open referrals or cases at once without things falling through the cracks.
· Comfortable being the go-between for clinical urgency (physician, discharge planner) and sales/account priorities.
· Comfortable making outbound phone calls directly to patients, families, and clinical staff - not just email/admin work.
· Manage a high volume of concurrent referrals/cases while maintaining attention to detail and follow-through.
Preferred Qualifications:
· Has hands-on experience navigating EMR/health system portals to independently locate discharge summaries, orders, and referral paperwork.
· Example EMR: Homecare Homebase, WellSky, Epic, Cerner, hospital case management portals
