We're hiring compassionate Home Health Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) in Macomb, Wayne and Washtenaw counties to provide personalized, one-on-one care that helps patients regain strength, improve mobility, reduce pain, and safely return to the activities they love. 💙 You Might Be a Great Fit If You:

Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence.
Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.
Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.
Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.

Requirements

💼 What We're Looking For:

Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.
Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.
Home health experience preferred (minimum one year).
Strong clinical judgment, critical thinking, and problem-solving skills.
Excellent communication, interpersonal, and patient education skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
Reliable transportation, a valid driver's license, and current auto insurance.

We were established in 1997 as Alacrity Staffing to provide healthcare personnel to hospitals, nursing facilities, and other medical entities throughout the country. As time went on, our focus shifted from facility-based staffing to personalized one on one care at home because we recognized the need for a smooth and safe transition from the hospital/facility setting to care in the home and our name changed to AdvisaCare Home Health. AdvisaCare became the solution for individuals and their families seeking superior home care services delivered with the exceptional customer service they deserve.

Home Care is personal and our founder, Kris Skogen, believes it should be local too. To that end, we employ a network of clinicians and healthcare staff that live in and understand the communities we serve. AdvisaCare is committed to supporting these communities through a variety of charitable endeavors.
