Position Overview: 

The Therapy Team Lead provides direct patient care while managing an assigned team of Therapists at their designated CHC site. The Team Lead serves as the first point of contact for clinical and operational issues, monitoring performance metrics such as schedule utilization, encounter completion, documentation timeliness, and collaborating with cross-functional leadership to align team performance with organizational goals. The Team Lead also supports onboarding, provider engagement, and workflow optimization to ensure effective integration and consistency across the assigned team. While the position has direct oversight of a team of Therapists, the position may also assist with projects and operations within other service lines as needed. This role balances clinical responsibilities with leadership duties focused on provider support, quality assurance, and operational efficiency.  

Key Areas of Ownership:  

• Provide direct patient care within the Ascend model, maintaining a clinical schedule that balances administrative duties, as determined by organizational leadership. The team lead position is slated for 80% to 85% direct patient care,15% to 20% administrative responsibilities. This is subject to change as the company needs evolve.  
• Oversight of assigned Therapy direct reports by monitoring productivity, documentation timeliness, and schedule utilization; serve as the first point of escalation for operational efficiencies.  
• Support onboarding initiatives and integration of new Therapists. 
• Provide proactive coaching, conduct bi-annual performance evaluations, and assist with professional development opportunities, ensuring a consistent level of accountability is maintained related to patient impact and quality of care. 
• Address routine clinical questions when appropriate, while ensuring providers follow supervision and CEED guidance channels. 
• Participate in training, orientation, and quality assurance initiatives, including chart reviews, to ensure documentation is accurate and meets regulatory compliance. 
• Contribute to policy and procedure development, compliance initiatives, and timely resolution of patient complaints. 
• Coordinate with Clinical Operations, CEED, Clinical Supervisors and Therapy leadership to optimize interdisciplinary workflows and training programs. 
• Perform additional leadership and operational responsibilities as assigned. 

Qualifications:  

• Active and unrestricted independent license (LCSW, LPC, LMFT) in state of primary residence and state of therapy services being rendered. (required) 
• 3+ years of clinical experience in behavioral health (required) 
• Board-certified to provide clinical supervision (preferred) 
• Prior leadership experience (preferred) 
• Prior policy and procedure development experience (preferred)

Position Summary

The Radiation Therapist is responsible for assisting with the daily patient flow in the clinic and supporting the Radiation Oncologist in delivering prescribed treatments. This position requires technical expertise in operating advanced radiation therapy equipment, ensuring treatment accuracy, and providing exceptional patient care in accordance with established procedures and safety standards.

Key Responsibilities

• Administer prescribed radiation treatments utilizing CyberKnife, TrueBeam, and Gammex HDR systems in conjunction with the Aria Record and Verify
• Perform patient simulations using the GE Lightspeed Big Bore CT under the supervision and direction of the Radiation Oncologist.
• Construct custom treatment devices to ensure precision and reproducibility of treatments.
• Collaborate closely with the on-site Dosimetrist and Medical Physicist to support treatment planning and delivery.
• Prepare treatment rooms, explain procedures to patients, position patients, and administer therapy while continuously monitoring the patient’s condition.
• Uphold the highest standards of patient safety, accuracy, and quality of care throughout all procedures.
• Maintain detailed treatment records in compliance with regulatory and departmental standards.

Qualifications

• Graduation from an accredited Radiation Therapy program.
• Current ARRT (T) certification and state licensure (as required).
• Current BLS certification.
• Minimum of 1 year of clinical experience in radiation therapy preferred (new graduates will be considered).
• Proficiency in the use of CyberKnife, TrueBeam, Aria, and GE CT systems is highly desirable.

 Qualifications and Requirements

• Current licensure as a Physical or Occupational Therapist (Sorry, No PTAs or COTAs).
• Malpractice/Liability Insurance (may be purchased at the time of job offer).
• Ability to lift up to 30 pounds, navigate stairs and varied home environments, and perform physical assessments as needed
• Reliable transportation and the ability to travel to and from member homes
• Proficient with technology: able to navigate online portals, upload documents, and use your preferred device (computer, tablet, or smartphone) for all work tasks
• Strong critical thinking, problem-solving, and communication skills

Preferred Experience (Not Required)

• Home safety assessment background
• Knowledge (Familiarity) of durable medical equipment
• Experience with home modifications
• Work history with diverse medical conditions

Position Summary:

We are looking for a Bilingual Licensed Therapist or Counselor (LPC, LCSW, LMFT) to provide counseling and therapy treatment to our patients. Services will be provided remotely, through telemedicine, to patients of our Community Health Center partners throughout Arizona. We are a "say yes" first clinical provider and look to treat patients where they are and guide them along their journey to recovery and stabilization.

*Clinician must be active or willing to obtain an Arizona License in addition to holding an active license in state of primary residence

*Clinician must be bilingual (English/ Spanish)

Key Areas of Ownership:

• Provides patient care modalities of evidence-based practices to best meet the clinical needs of the patients. Examples include; CBT, DBT, Solution-Focused Therapy, Psychoeducation (Symptom Recognition), Trauma Therapy, Addiction Therapy, Acceptance and Commitment Therapy, Bereavement/Grief Counseling, etc.
• These specific treatment modalities will be implemented in both the individual and group therapy setting, with a focus of engaging patients in their treatment plan.
• Review and signature of patient clinical Psychosocial assessment, diagnosis, treatment plan, and consents as needed for the patient in a timely manner. Maintenance of the clinic record, including documentation of activities performed as part of the service delivery process.
• Documents all services and patient activities appropriately. Ensures documentation complies with best clinical practices, contracts, regulatory and other legal
• Conducts crisis intervention for the individual and family based on the individual’s safety plan.
• Participate in regular case planning with the involved Multidisciplinary Team as new information is gathered to ensure the most clinically appropriate patient disposition.
• Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor
• Provides case management services to promote recovery and community reintegration as needed.
• Performs other duties as assigned

Qualifications:  

• Active, unrestricted mental health license in state of primary residence and state of medical services being provided (Arizona)
• Comfortable working in a remote environment

*Clinician must be active or willing to obtain an Arizona License in addition to holding an active license in state of primary residence

*Clinician must be willing to work evening hours, either Mon-Fri 10am-8pm or Tues-Fri 10am-8pm (2 days per week minimum)
