Position Summary

We are seeking a compassionate and organized Social Services Assistant to support our Social Services Department in meeting the emotional, psychosocial, and discharge planning needs of our residents. This role works closely with residents, families, community agencies, and the interdisciplinary team to help ensure residents receive the support and resources necessary to achieve their highest level of well-being.

Key Responsibilities

  • Assist the Social Services Director in planning, organizing, implementing, and evaluating social service programs
  • Support residents in achieving the highest possible level of independence and quality of life
  • Assist with identifying and addressing residents' psychosocial and emotional needs
  • Meet with administration, nursing staff, medical providers, and other departments to coordinate resident services
  • Maintain accurate social service documentation, records, and progress notes
  • Assist residents and families with information regarding Medicare, Medicaid, and other available resources
  • Coordinate social service activities with other departments as needed
  • Assist residents and families in coping with adjustment issues, illness, disabilities, and long-term care placement
  • Help secure community resources and referrals to meet resident needs
  • Assist with arranging outpatient appointments and ancillary services, including dental, podiatry, psychiatric, and optometry services
  • Participate in discharge planning and coordinate referrals for equipment, services, and community agencies
  • Develop and maintain positive relationships with outside healthcare, social service, and community organizations
  • Participate in orientation and in-service education programs
  • Assist with department projects, budgeting tasks, and administrative responsibilities as assigned
  • Maintain HIPAA compliance and protect all resident information

Qualifications

Education & Experience

  • High school diploma or equivalent required
  • Associate's or Bachelor's degree preferred, but not required
  • Prior healthcare experience preferred
  • Experience in social services, discharge planning, case management, customer service, or long-term care is a plus
