POSITION SUMMARY

The Children’s Hospital of Eastern Ontario (CHEO) Extensive Needs Service (ENS) is a service developed to address the needs of children and youth whose extensive and complex needs surpass the capacity of our existing healthcare system. The ENS brings a multidisciplinary team around a family to rapidly implement an intervention plan aimed at addressing the needs of the family as a whole. We are building a dedicated pathway for the most vulnerable children and youth up to 18 years of age with complex needs.

MAIN RESPONSIBILITIES

  • Conduct physical assessments during clinic appointments to support medical and psychiatric evaluations.
  • Perform health surveillance by triaging referrals, gathering medical histories, and identifying unmet health needs during pre-clinic nursing assessments for the medical clinic, and in case review for Psychiatry consultations.
  • Coordinate case management for medical clinic patients, ensuring continuity of care and monitoring outcomes of medical interventions.
  • Advocate for access to community care and resources, facilitating connections to appropriate supports and services.
  • Administer seasonal vaccinations (e.g., influenza) and support other preventative health measures within the clinic setting.
  • Provide health education to patients and families and collaboratively develop and implement safety plans with families and care teams.
  • Support patients during clinic visits by managing interfering behaviors and ensuring a safe, therapeutic environment at all times.
  • Maintain documentation of clinical interactions, family discussions, and safety incidents.
  • Collaborate with interdisciplinary team members including physicians, psychiatrists, and allied health professionals to align care with ENS goals, including those of the feeding pathway.

QUALIFICATIONS

  • Bachelor's of Science in Nursing (BScN) (Preferred)
  • Minimum two (2) years Mental Health and neurodevelopmental health experience with the pediatric population (Essential)
  • Current Registration with the College of Nurses of Ontario (CNO) (Essential)
  • Current Cardiopulmonary Resuscitation (CPR) certification (Essential)
  • Demonstrated strong communication and interpersonal skills (Essential)
  • Demonstrated culturally sensitive approach to nursing care (Essential)
  • Strong family centered care skills (Essential)
  • Possibility of occasional travel to Cornwall and Renfrew (Essential)
  • Vulnerable Sector Check (Level 3) (Essential)
  • Bilingualism (French/English) (Preferred)

This assignment may be extended or shortened based on operational needs.

Effective March 29, 2021, CHEO will conduct a more comprehensive pre-screening and evaluation process. This process may include more varied and multidimensional testing such as a written test, practical exam, or other assessment. All candidates who meet the essential and preferred qualifications may be tested. Bilingualism (French/English) will be validated by a language proficiency assessment conducted by a third party provider.
