Occupational Therapist, Home Based Care, Southern Sydney

This role is 100% home based, supporting people in aged care and people living with disability across Sydney's southern suburbs. You'll be joining a mobile allied health provider with an established, collaborative multidisciplinary team.

About the role

• 100% adult caseload, made up of aged care (Support at Home / CHSP) and NDIS disability support
• Home safety assessments, assistive technology prescription, home modifications and functional capacity reporting
• Appointments scheduled within roughly a 30 minute drive of your own home base
• Home based role, with occasional access to our clinic offices for clinic based work if needed
• Monthly in person team meetings held at one of our clinic locations

Key responsibilities

• Conduct comprehensive assessments and develop individualised intervention plans for adult clients
• Prescribe and review assistive technology, equipment and home modifications
• Prepare clear, well documented reports and recommendations
• Liaise with support coordinators, case managers, families and other allied health providers
• Contribute to a collaborative multidisciplinary team alongside Physiotherapists, Exercise Physiologists, Speech Pathologists and fellow OTs

The team

• You'll join a wider Occupational Therapy and Speech Pathology team of around 10 clinicians
• The broader clinical team includes around 12 Physiotherapists and Exercise Physiologists, supported by 3 administrative staff who manage scheduling and non-clinical logistics
• The full team meets fortnightly, alternating between in person and online sessions

Requirements

• Degree in Occupational Therapy with current AHPRA registration
• Comfortable working autonomously in clients' homes, with sound clinical reasoning and reporting skills
• Full time preferred, though 3 to 4 days a week can be considered for the right candidate
• Own car and driver's licence

What's on offer

• $80,000 to $85,000 base salary, depending on experience
• $1.01 per km car allowance, plus tolls and parking reimbursed (no car is provided, but running costs are fully covered)
