Diablo Valley Post Acute is seeking a dedicated, experienced, and compassionate Licensed Vocational Nurse (LVN) Supervisor to join our clinical leadership team.

The LVN Supervisor will help oversee daily nursing operations, support nursing staff, promote quality resident care, and ensure compliance with facility policies and applicable regulations. Position Details

  • Position: LVN Supervisor

  • Employment Type: Full-Time

  • Pay Rate: $38.00–$39.14 per hour

  • Location: Diablo Valley Post Acute

Responsibilities

  • Supervise and support nursing staff to ensure quality resident care

  • Oversee daily nursing activities and assist with coordinating resident care

  • Monitor residents' conditions and ensure changes are promptly addressed and reported

  • Assist with medication administration, treatments, assessments, and other nursing duties as needed

  • Provide guidance, direction, and support to CNAs and other nursing team members

  • Ensure individualized care plans are followed and updated as appropriate

  • Review nursing documentation for accuracy, completeness, and compliance

  • Communicate effectively with physicians, residents, families, and interdisciplinary team members

  • Assist with staff training, orientation, and ongoing education

  • Promote adherence to infection prevention and control practices

  • Help ensure compliance with facility policies, state and federal regulations, and nursing standards

  • Address resident and family concerns professionally and promptly

  • Assist with quality assurance and performance improvement initiatives

  • Support the Director of Nursing and facility leadership with clinical and operational needs

  • Respond appropriately to emergencies and changes in resident condition

  • Perform other duties as assigned

Qualifications

  • Current, active California LVN license required

  • Previous skilled nursing or post-acute care experience preferred

  • Prior supervisory or charge nurse experience preferred

  • Strong clinical assessment and critical-thinking skills

  • Excellent leadership, communication, and interpersonal skills

  • Ability to effectively supervise, motivate, and support nursing staff

  • Strong organizational and time-management abilities

  • Knowledge of skilled nursing regulations and best practices

  • Compassionate, professional, and resident-centered approach

  • Current CPR certification preferred
