Roseville Care Center is looking for a compassionate, dependable, and team-oriented Licensed Vocational Nurse (LVN) to join our nursing team on a part-time basis.

The LVN provides direct nursing care to residents while supervising and supporting nursing assistants and ensuring care is delivered in accordance with applicable federal, state, and local regulations. The ideal candidate is a positive leader who takes pride in providing quality resident care and creating a calm, respectful environment.

What You’ll Do

Resident Care

  • Provide direct nursing care to residents according to their care plans and individual needs.
  • Monitor residents' physical and emotional condition and report changes promptly.
  • Administer medications and treatments as ordered.
  • Perform nursing procedures such as catheterization, tube feedings, suctioning, dressing changes, specimen collection, and other services within the LVN scope of practice.
  • Take and record vital signs and monitor residents as needed.
  • Notify physicians and responsible parties of significant changes in resident condition, accidents, or incidents.
  • Participate in admissions, transfers, and discharges.
  • Work collaboratively with physicians, rehabilitation, dietary, social services, activities, and other departments.

Nursing Documentation & Care Planning

  • Complete accurate and timely nursing documentation and charting.
  • Transcribe and implement physician orders as required.
  • Document resident care, treatments, incidents, and responses to care.
  • Review resident care plans and communicate needed changes to the Nurse Supervisor.
  • Ensure nursing care and treatments are provided according to resident care plans and preferences.
  • Assist with MDS and care plan activities as requested.

Leadership & Supervision

  • Provide direction and leadership to nursing assistants and other assigned nursing personnel.
  • Make rounds to ensure nursing services are being provided according to acceptable nursing standards.
  • Develop and communicate work assignments.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Promote teamwork and maintain a positive, respectful, and calm environment.
  • Assist with identifying staffing needs and communicating staffing concerns to the Nurse Supervisor.
  • Help ensure staff follow facility policies, procedures, safety requirements, and infection-control practices.

Medication Administration

  • Prepare and administer medications according to physician orders and facility procedures.
  • Verify resident identity before administering medications and treatments.
  • Maintain accurate narcotic and medication records.
  • Report medication discrepancies to the Nurse Supervisor.
  • Ensure appropriate medication, supply, and equipment levels are maintained.

Safety & Infection Control

  • Follow and promote established infection-control, safety, and sanitation procedures.
  • Maintain clean and safe nursing areas.
  • Ensure appropriate use of personal protective equipment.
  • Follow proper hand hygiene and isolation precautions.
  • Participate in emergency, fire, and disaster preparedness procedures.
  • Report hazardous conditions, occupational exposures, and safety concerns promptly.

Staff Development

  • Participate in required facility in-services and continuing education.
  • Assist with orientation and training of nursing personnel as appropriate.
  • Maintain current knowledge of nursing practices, regulations, and facility procedures.
  • Maintain an active nursing license and required certifications.

What We’re Looking For

  • Graduate of an approved LVN/LPN program or possess a nursing degree from an accredited college or university.
  • Current, active, and unencumbered California LVN license.
  • Current CPR certification.
  • Knowledge and experience with PCC preferred.
  • Ability to read, write, speak, and understand English.
  • Knowledge of nursing practices, procedures, and regulations applicable to long-term care.
  • Strong communication, organizational, leadership, and decision-making skills.
  • Ability to work effectively with residents, families, nursing staff, physicians, and other healthcare professionals.
  • Compassionate, patient, positive, and professional approach to resident care.
  • Ability to work independently while contributing effectively as part of a team.
  • Must maintain all required licensing and continuing education and remain in good standing with the California Board of Nursing.

Physical Requirements

The LVN must be able to move intermittently throughout the workday and perform the physical requirements necessary to provide resident care. The position may require pushing, pulling, moving, and/or carrying up to 25 pounds to a minimum height of 5 feet and a minimum distance of 50 feet.

The employee must be able to communicate effectively, cope with the mental and emotional demands of the position, and work effectively with elderly, ill, disabled, emotionally upset, and occasionally hostile individuals.

The LVN may be required to assist with the evacuation of residents during emergency situations.
