Job Description:
Licensed Vocational Nurse Clinical Coordinator
Bring your nursing experience into a collaborative clinical coordination role where you can support patients, caregivers, and field teams—while working primarily in an office setting.

What You’ll Do:
Reviewing and clearing Homecare Homebase (HCHB) coordination notes
Processing administrative and clinical workflows accurately and on time
Obtaining verbal, written, and faxed physician orders and entering them for review and approval
Reviewing, updating, and maintaining the At-Risk Registry
Assisting with patient follow-up appointments, health summaries, pre-authorizations, and patient questions
Completing chart audits according to Elara Caring policies
Verifying receipt of Medicare Notice of Non-Coverage documentation
Protecting patient and employee privacy in accordance with HIPAA requirements
Supporting the branch with occasional field visits and direct patient care, as needed
Completing additional clinical or administrative projects as assigned
Bilingual Spanish and English skills are a plus!
What You’ll Need
Graduation from an accredited school of nursing
Current, unrestricted LVN license valid in the state of employment
At least one year of nursing experience in a home health setting
Strong organization, communication, and attention to detail
Ability to manage multiple priorities in a fast-paced environment
